Position Summary: 

The Transformative Case Management Program is designed to follow participants on their journey 
of transformation, using a person-led framework that addresses needs holistically. The program

Worker will work primarily with shelter residents, developing holistic case plans, completing 
assessments, and implementing, monitoring, and evaluating actions required to meet plan

 • Providing services to residents using evidence-based case management approaches, such
