The Personal Support Worker will support, as assigned, the needs of the vulnerable, at risk, older adults and seniors living in the multi-sites under the auspices of the program. This support relates to the physical, cognitive, mental health and social/recreational needs of the clients. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The Personal Support Worker will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.&nbsp; Back to Home Penetanguishene LOFT provides a compassionate and comprehensive assisted living solution for older adults grappling with complex mental and physical health challenges, including substance use, homelessness, dementia, and other neurological disorders. Our services cater to seniors aged 55 and above who require permanent subsidized housing with specialized assistance. Eligibility includes those designated as Alternative Level of Care (ALC) or at risk of being designated, and who need around-the-clock support to remain safely at home. Referrals are managed through Centralized Intake, with sources typically being hospitals in the North Simcoe Muskoka (NSM) region. Once referred, potential residents undergo an intake process by LOFT, which includes a facility tour and acceptance of the housing offer. Our facility possesses 27 beds, comprising 24 permanent and 3 transitional options, offering both private and shared accommodations. Additionally, we provide Hub and Spoke services, which are community-based support designed for those who, while able to live at home, need ongoing care with personal supports available 24/7. This system is supported through our existing Assisted Living buildings, ensuring that clients receive crucial support while remaining in their own homes. For those under 55 requiring significant assistance with activities of daily living (ADLs), we consider cases on an individual basis, ensuring that every resident’s unique needs are met with tailored care.What You Will Do<ul><li>Support of the physical and mental health requirements of client<ul><li>Establish a working rapport with clients that supports their dignity, encourages personal empowerment and facilitates addressing their psychosocial, behaviour management and medical needs.</li><li>Support clients with reminders related to taking their medications at the scheduled time(s) and in accordance with their physicians' instructions.</li><li>Adhere to residence/program established medication system procedures, including ordering, recording and reminding clients about the administration of their medication as well as the self- medication program.</li><li>Monitor and support clients with medical problems and illnesses, including cleaning areas which require immediate attention, i.e. linens, floor coverings, etc. for the comfort and support of the clients. Further, record illnesses and circumstances related to illness, and arrange for medical/emergency intervention in liaison with the Senior PSW and Program Manager.</li><li>Encourage and assist clients in the following activities of daily living:<ul><li>laundering and ironing of personal clothing</li><li>dressing/undressing, and ensure clients are dressed appropriately,</li><li>personal hygiene including grooming, shaving, oral hygiene, nails and bathing.</li><li>ambulation, toileting and transferring support,</li><li>at mealtimes, meal preparation, serving, feeding, cutting food, etc.,</li><li>care for incontinent clients, make client's bed,</li><li>shopping.</li></ul></li><li>As a means of promoting a normalized lifestyle, support clients with daily living chores such as bed making, dusting, sweeping and generally keeping their living space in a clean and orderly fashion.</li><li>Provide support with managing behaviours and problems arising from cognitive issues.</li><li>Provide social support in areas such as conversation, making phone calls, writing letters, facilitating interest in hobbies and other recreational activities.</li><li>Escort to medical appointments, visiting friends and other identified needs for social interaction in the community as needed.</li><li>Maintain current and accurate records of the provision of care services and document behaviours for individual clients in accordance with established procedures.</li><li>Participate in regularly scheduled meetings with clients and/or other members of the staff team for the purpose of developing goals of support for individual clients. Position Overview: 

As a member of an interdisciplinary team, the Medical Secretary provides reception and 
clerical support to the staff and programs at Vibrant and is responsible for receiving and 
welcoming clients and guests to our Health Centre. Duties include booking/rescheduling 
appointments, monitoring clinician schedules, and providing general administrative support 
to primary care &amp; allied care providers using an electronic medical record (EMR). Some 
evening work will be required. This position is expected to work onsite. 
 
Summary of Duties 

 • Provide reception services including positive customer service, communication with 
clients, answering incoming calls and directing calls accordingly 

 • Respond to general inquiries, obtaining services for clients (e.g. taxis, referrals to 
other agencies etc.) and providing information to the public and other agencies 
regarding services 

 • Scan, fax and / or mail documents as required 
 • Triage incoming phone calls; schedule &amp; confirm client appointments, tests, follow-

ups, and manage schedules using an electronic medical record (EMR) as required 
 • Respond to the administrative needs of clinical staff by assisting with computer work, 

photocopying, faxing, filing, and scheduling 


 • Ensure client records and files are accurate and current, and update the medical 
record system with appropriate information as needed 

 • Coordinate referrals with external service providers 
 • Ensure clients have appropriate registration and requisition forms 
 • Distribute harm reduction supplies when needed 
 • Share information collected from clients with the appropriate clinical staff 
 • Participate in meetings as required 
 • Adhere to all policies and procedures of the Vibrant Community Health 
 • Participate in other duties as assigned 

Qualifications/Skills 

 • High School Diploma or equivalent, medical administrative education an asset 
 • Minimum 1 years’ experience as a medical secretary or administrative assistant 
 • Experience working in a community health setting 
 • Proficiency in the use of computer technology, specifically with using electronic 

medical record (EMR) systems; experience using PS Suite is an asset 
 • Knowledge of the health care sector including primary health care and the 

community-based health model 
 • Demonstrated experience working with vulnerable populations with complex health 

needs including seniors, people living with disabilities and those who may have 
difficulty accessing health care 

 • Knowledge of harm reduction principles and Independent Living philosophy 
considered an asset 

 • Demonstrated ability to work independently and as a member of a multi-disciplinary 
team. 

 • Excellent communication, interpersonal and organizational skills 
 • Police clearance documentation will be required for employment

Working from a harm reduction and trauma-informed perspective, the PSW-SCC will be part of an interdisciplinary team of medical professionals, community partners, follow-up workers, harm reduction workers and peer support workers. As a Personal Support Worker (PSW) at the SCC, you will play a crucial role in providing compassionate and client-centered care to individuals in a non-traditional setting. Working within a harm reduction and trauma-informed framework, you will support clients with daily living activities, basic personal care, and connection to essential services while fostering an environment that promotes dignity, respect, and stability. The ideal candidate for this position will embody the philosophies of our department and peer program, adhering diligently to all policies, procedures, and requirements. Reporting directly to the Supervisor, Stabilization and Connection Centre you will be an integral part of our team. MAJOR DUTIES: • Provide personal support. Such services include but are not limited to: grooming, bathing, dressing, feeding, peri-care, dental care, laundry, lift and transfer, etc. • Work in alignment with harm reduction principles, recognizing and respecting clients' autonomy and lived experiences. • Assist in monitoring client well-being, identifying concerns, and escalating situations when necessary to the team on site. • Work collaboratively and develop strong partnerships with co-workers, service providers and community partners • Understand/facilitate the emergency plan and fire plan and respond professionally to all client/program emergencies. Attend mandatory training, health and safety and fire drills. • Collaborate with case managers, nurses, peer workers, and other team members to ensure a holistic and client-centered approach to care. • Report any physical and/or emotional changes in a client or any changes in service requirements to the Manager or Supervisor. • Maintain up-to-date, accurate written/entered electronic data information, reports, and other correspondence as needed • Engage and build respectful and professional relationships with co-workers and community partners • Attend regular team meetings • Keep up to date on gerontological issues and health care, and take relevant courses, (e.g. mental health, medication, C.P.R., WHMIS, Food Handling and First Aid), as directed.
