Position Summary:The&nbsp;Intensive Housing Case Manager&nbsp;is responsible for fulfillment of the Intensive case management and housing responsibilities of the&nbsp;Intensive Housing Case Management Services Program&nbsp;within Peel Region. The program uses a collaborative approach across community, health and social service sectors in the development and implementation of individual plans of care that promote improved wellbeing for clients who are homeless or at risk of homelessness.
