These complex challenges could include mental health and/or addiction challenges, cognitive impairment, ongoing physical health issues, responsive behaviours and/or other neurological disorders. LOFT’s Supportive Housing Services in York Region/South Simcoe provide a variety of housing options to adults (16 years+), either within LOFT owned houses/buildings or through head lease arrangements with private landlords. We offer a range of programming that meets the level of support needs or unique challenges of specific populations. These include small group living arrangements for those requiring minimal support, individual and shared accommodations, and staffed congregate settings for individuals needing daily support. Within that continuum, we also serve people with current involvement in the criminal justice system or those with several comorbid health issues. POSITION SUMMARYTAIBU’s Diabetes Education Program consists of a specialized inter-professional team with a focus on providing Diabetes Education and services for residents of Malvern and the Black Community across the GTA.The Registered Diabetes Nurse Educator functions interdependently with other members of the Diabetes Education Program, clinical and community program teams within a health promotion framework. The Nurse Educator provides education to clients through individual and group counseling to assist in the transference of knowledge to effectively self manage their diabetes. The Nurse Educator will possess a sound knowledge base around the prevention, care and management of diabetes. The service is a 7 day a week mobile crisis intervention and outreach service focused on individuals 65 years of age and older who have a mental health and/or substance use issues, with some exceptions for individuals 55 - 64 years of age, including those who are homeless.&nbsp; The service area is the City of Toronto. The population of focus for this service is frail, marginalized, at risk seniors, who may be homeless or vulnerably housed, living in poverty, and/or new immigrants.&nbsp;&nbsp; This position is responsible to WoodGreen’s Crisis Supervisor.&nbsp;&nbsp; This position is expected to carry out the services in accordance to WoodGreen’s policies and procedures, COSS’ mandate and procedures, Ministry of Health, and health related authorities, and all relevant legislation.
