Case Managers individually and/ or as part of a team that may include a Coordinator, Peer Support Workers, Housing Support Workers, and other members of interdisciplinary teams to meet clients’ housing, health and other social needs. The program works closely to assess, plan, treat and support clients as they work toward recovery. The position may involve driving and a valid Ontario driver’s license and insurance are required.&nbsp;&nbsp; M-DOT is a specialized team of providers from LOFT, Inner City Health Associates (ICHA) and the Centre for Addictions and Mental Health (CAMH). M-DOT provides clinical hands-on support to those with complex healthcare needs, who are experiencing chronic homelessness. M-DOT provides outreach, case management and peer support services with access to psychiatric and medical assessment, consultation, access to treatment, service planning and advocacy. Services are designed to be short-term, focused on comprehensive assessment, stabilization and linking to long term resources. M-DOT provides services to individuals often identified as ‘complex’, who have not responded positively to other programs or attempts to engage. The aim of M-DOT is to provide a low barrier transitional service, which meets individuals where they are at and assists in stabilizing their health, mental health and substance use.&nbsp; M-DOT works from a housing first and harm reduction approach. Founded in 1960, the Youth Services Bureau of Ottawa (YSB) is one of the largest and most

the Coordinator is responsible for the overall functioning and operation of the Downtown Services and

Principal Duties: 


Staff &amp; Program Related: 

 • Support the effective recruitment, supervision, evaluation and training of unit staff and student 

placements as assigned 

 • Oversee and ensure the smooth day to day operations of the unit including the delegation of and 

coordination of essential daily activities

 • Superviser et assurer le bon fonctionnement quotidien de l’unité, y compris la délégation et la 

coordination des activités quotidiennes essentielles. 

 • Assurer la préparation ponctuelle des horaires du personnel. 

 • Animer les réunions du personnel de l’unité afin d’évaluer les besoins et les résultats du 

programme et des clients. 

 • Soutenir le personnel de mobilisation des jeunes pour qu’il travaille efficacement avec les 

équipes de jeunes afin d’atteindre les objectifs du plan de travail. 

 • Contribuer au développement continu de relations de travail, de processus et de procédures 

efficaces entre pairs aidants et personnel adulte. 

 • Poursuivre le développement de programmes par les jeunes et pour les jeunes au sein du centre 

d’accueil. 

 • Contribuer à l’intégration continue des partenaires de services sur place (personnel de la 

clinique, personnel scolaire, gestion de cas Wabano) aux services du centre d’accueil. 

 • Attribuer les tâches au personnel, selon les besoins. 

 • Assurer la liaison avec les autres services du Bureau des services à la jeunesse et de la 

communauté, et participer aux réunions, au besoin. 

 • Contribuer à l’évaluation et au développement des services, notamment en identifiant les 

tendances dans le domaine. 

 • Contribuer aux activités et aux rapports d’assurance de la qualité, et participer aux activités de 

recherche, au besoin. 

 • Collaborer avec le directeur du programme et les bailleurs de fonds sur les activités liées au 

programme. 

 • Des services sur appel réguliers ou périodiques sont requis. 

 • Veiller au respect des politiques et procédures de l’agence, des politiques du programme, de la 

convention collective et de la législation en vigueur. 

 • Veiller au respect des pratiques de travail sécuritaires et à leur mise en place par le Bureau des 

services à la jeunesse et le programme. 

 • Coordonner toutes les activités de projet assignées. 

Clinique/Client 

 • Superviser et participer à la planification des admissions et des sorties, selon les besoins. 

 • Superviser les évaluations des risques imminents et s’assurer que des plans de sécurité et de 

suivi appropriés sont en place. 


 • Fournir des services directs aux clients, selon les besoins. 

 • Superviser la planification des célébrations et des événements spéciaux définis par le 

programme. 

 • Superviser l’accès et l’orientation vers les services appropriés au sein du Bureau des services à la 

jeunesse et de la communauté. 

 Services centraux/Administration : 

 • S’assurer que toutes les tâches administratives sont effectuées dans les délais et conformément 

aux politiques de l’unité et du Bureau des services à la jeunesse, y compris la préparation des 

horaires, les résumés des quarts de travail, les rapports d’anomalies, la soumission des paies, 

l’approbation des demandes de congés et la saisie des données. 

 • Préparer les rapports statistiques ou autres rapports requis par le directeur ou les bailleurs de 

fonds du programme. 

 • S’assurer que les fichiers électroniques sont tenus à jour et conformes aux directives établies. 

 • Soutenir la gestion du budget et être responsable des dépenses. 

 • Préparer les demandes de licences et d’agrément, le cas échéant. 

 • Tenir à jour l’inventaire de fournitures. 

Description du poste/Qualifications :

 • Diplôme en travail social ou dans un domaine connexe 

 • Minimum de cinq ans d’expérience dans un milieu connexe 

 • Minimum de deux ans d’expérience dans un rôle de supervision 

 • Connaissance des services d’accueil et du programme Logement d’abord pour les jeunes 

 • Compétences organisationnelles et administratives avérées 

 • Excellentes compétences en communication écrite et orale 

 • Compétences avérées en prise de décision et en résolution de problèmes 

 • Expérience en création et en présentation de présentations 

 • Expérience en gestion budgétaire 

 • Connaissance des ministères, des lignes directrices, des lois, des règlements et des exigences 

 • Connaissance des programmes et activités du Bureau des services à la jeunesse, ainsi que des 

politiques et procédures 


 • Connaissance des partenariats communautaires et des services offerts 

 • Compétences informatiques 

 • Expérience de travail dans un milieu à but non lucratif 

 • Connaissance des problèmes de santé mentale chez les adolescents 

 • Expérience des problèmes de comportement 

 • Connaissance de diverses approches thérapeutiques ainsi que des stratégies et techniques 

d’intervention 

 • Disponibilité à travailler selon un horaire flexible et à assumer des responsabilités sur appel, au 

besoin 

 • La maîtrise de l’anglais et du français, à l’oral comme à l’écrit, est essentielle
