The SNAP Program groups specifically serve children ages 6-11 yrs old and their families, particularly those from Black and racialized backgrounds, newcomers, and those living in low-income or high-risk situations. Additionally, we provide crisis support for other individuals within our community. Our work includes culturally responsive best practices and interventions, designed to empower individuals and families and promote healthy children's development. Most importantly, the SNAP program provides support focusing on behavioral and emotional regulation and modification. Supports are provided through the channels of advocacy, case management, crisis counseling, parental &amp; children group programming, mentorship/sponsorship, resource navigation, needs replenishment and collaboration with other appropriate stakeholders and community partners Successful candidates will be expected to work flexible hours, in accessible locations to accommodate their clients’ needs. Job Purpose: 
 

As a dynamic and welcoming member of our team, the Case Management Worker will provide intake 
interviews and follow-up support services with new members. This position provides front line case 
management and counseling to new members, refers them to appropriate programs and resources and 
provides ongoing follow up support, in addition to providing group facilitation for wellness programs 
and emotional support groups. 

 
Reports to: 
Executive Director 

 
Primary Responsibilities: 
Intake Procedure/Support Services 
 • Provide Intake interviews for new adult members. 
 • Coordinates individual follow up appointments, member outreach and support for drop-in visitors. 
 • Ensures that membership and intake forms are completed, as well as all relevant referrals to peer 

support and program registrations. 
 • Manage an individual caseload providing individual emotional support to families and individuals as 

required. 
 • Provide support and resources for Caregivers. 
 • Facilitate group programming or seminars as requested by the Program Coordinator and Executive 

Director. 
 • Collects statistics for services provided including providing a monthly report to the Board of 

Directors. 
 

Follow-Up Procedure 
 • Follow procedures to track follow-up phone calls, support needed after a group finishes etc. to 

ensure accountability, continued support and member satisfaction. 
 • Ensure client notes and database entries are conducted in a timely manner and written in clear, 

concise, and non-judgmental language. 
 
 

Resources 
 • Compile, create and expand resources for members as requested by the Program Coordinator or 

Executive Director. 
 • Assist with needs assessment and development of peer support services, seminars and support 

groups as required. 
 
 

Qualifications: 
Experience 
 • 5-10 years of social work or human services counseling.

Position: Social worker Permanent Full Time 

About Across Boundaries: 

Across Boundaries is a mental health center that provides a range of mental health and addictions support and 

services to individuals from racialized and Black communities in the Greater Toronto Area. We have a holistic approach 

to mental health care and operate within an anti-racism/anti-oppression and resisting anti-Black racism frameworks. 

These frameworks focus on providing opportunities to respond to systemic barriers, advocate for the inclusion of 

traditionally and historically silenced voices at decision-making tables, and strive for change at individual, 

organizational and systemic levels. 

Job Summary: 

The Social Worker will report directly to the Program Manager. They will work collaboratively with various teams to 
provide prompt and appropriate access to services for racialized individuals from the community who are experiencing 
mental health and addictions issues. The worker will provide both intensive and non-intensive services to clients based 
on their needs and will have a caseload of up to 20 to 25 program participants. The Social Worker will be expected to 
work directly in the community using the agency van for outreach and support to community members. 
 
Normal Hours for full-time employees consist of five (5) shifts or eight (8) hours per day, Monday to Friday, and Saturdays 
on a rotating basis, inclusive of a thirty (30) minutes unpaid lunch break (entitled under the ESA) and an additional two 
(fifteen (15) minute unpaid rest breaks. Normal hours of work will generally be scheduled between 8:00 am and 8:00 
pm. Employees may be requested, but not required, to work some Sundays. 

Qualifications &amp; Responsibilities: 
 

 • Master’s degree in social work from a recognized university or equivalent. 

 • Knowledge of holistic, anti-racism/anti-oppression and resisting anti-Black racism frameworks as guiding 

principles for assessing and developing treatment plans for clients. 

 • Supervised clinical training. 

 • Practical experience in the use of Dialectical Behaviour Therapy (DBT) and Cognitive Behavioral Therapy (CBT) 

and able to provide psychotherapeutic assessments and structured counseling to individuals presenting with a 

wide range of mental health conditions. 

 • Clinical competence to provide structured counseling in the areas of trauma, depression, anxiety, grief/loss, 

and/or addictions. 

 • Experience with case management an asset. 

 • Demonstrated ability to produce detailed, high-quality assessment and treatment reports and clinical 

documentation. 

 • Able to communicate and work effectively in an interdisciplinary team environment and willingness to accept 

clinical direction. 

 • Familiarity with the Ontario Mental Health Act and Mental Health reform. 

 • Proficiency in assessment, psychotherapy, and psychosocial counselling techniques for working with 

individuals, groups, and communities. 

 • Demonstrated skill and competence in establishing supportive, trusting relationships with persons living with 

mental health and addictions issues. 

 • Familiarity with signs and symptoms of trauma, able to respond appropriately in making referrals and helping 

clients manage these symptoms. 

 • Experience working with clients with a concurrent disorder and those living in unstable housing situations or 

homelessness. 

 • Demonstrated ability to work independently, cooperatively, and constructively within a multidisciplinary team 

environment.

 • A valid holder of G license to drive a high roof, extended Ford Transit van. 

 • Clean driving record with minimum 3 years of insured driving experience

Salary Range: $65,010.16 to $66,040.46 per annum (pro-rated)
