In partnership with the Niagara Region Indigenous Community, Bethlehem Housing and Support Services (BHSS) will operate a Supportive Transitional Housing program for women and children in the city of Fort Erie.&nbsp; We are seeking two Community Service Navigators (CSN) who will&nbsp;provide advocacy and support to clients in the Transitional Housing Program to achieve community integration while working on a long-term plan to achieve housing stability in the community.&nbsp; &nbsp;The Navigator will work with Indigenous women and children, and those facing issues such as poverty, homelessness, domestic violence, mental health issues, addiction issues and/or family breakdown.&nbsp;In collaboration with the staff team, the program provides safe transitional housing, basic structure in independent living, support with goal setting, and personal capacity building programs.&nbsp; By creating and offering this supportive environment, clients and their families can overcome trauma, address the issues that have led to or kept them homeless, and begin to rebuild their lives through a support network.&nbsp;QUALIFICATIONS &amp; COMPETENCIES<ul><li>Diploma or Degree in Social Services or another public service-related field.</li><li>Minimum of two years in related social service experience, preferably with direct program delivery, frontline or client facing role in the areas of supportive housing, violence against women, mental health, homelessness, and/or addictions. As a dynamic and welcoming member of our team, the Case Management Worker will provide intake 
interviews and follow-up support services with new members. This position provides front line case 
management and counseling to new members, refers them to appropriate programs and resources and 
provides ongoing follow up support, in addition to providing group facilitation for wellness programs 
and emotional support groups.

Primary Responsibilities: 
Intake Procedure/Support Services 
 • Provide Intake interviews for new adult members. 
 • Coordinates individual follow up appointments, member outreach and support for drop-in visitors. 
 • Ensures that membership and intake forms are completed, as well as all relevant referrals to peer 

support and program registrations. 
 • Manage an individual caseload providing individual emotional support to families and individuals as

Job Summary: 

The Social Worker will report directly to the Program Manager. They will work collaboratively with various teams to 
provide prompt and appropriate access to services for racialized individuals from the community who are experiencing 
mental health and addictions issues. The worker will provide both intensive and non-intensive services to clients based 
on their needs and will have a caseload of up to 20 to 25 program participants. The Social Worker will be expected to 
work directly in the community using the agency van for outreach and support to community members. 
 
Normal Hours for full-time employees consist of five (5) shifts or eight (8) hours per day, Monday to Friday, and Saturdays 
on a rotating basis, inclusive of a thirty (30) minutes unpaid lunch break (entitled under the ESA) and an additional two 
(fifteen (15) minute unpaid rest breaks. Normal hours of work will generally be scheduled between 8:00 am and 8:00 
pm. Employees may be requested, but not required, to work some Sundays. 

Qualifications &amp; Responsibilities: 
 

 • Master’s degree in social work from a recognized university or equivalent. 

 • Knowledge of holistic, anti-racism/anti-oppression and resisting anti-Black racism frameworks as guiding 

principles for assessing and developing treatment plans for clients. 

 • Supervised clinical training. 

 • Practical experience in the use of Dialectical Behaviour Therapy (DBT) and Cognitive Behavioral Therapy (CBT) 

and able to provide psychotherapeutic assessments and structured counseling to individuals presenting with a 

wide range of mental health conditions. 

 • Clinical competence to provide structured counseling in the areas of trauma, depression, anxiety, grief/loss, 

and/or addictions. 

 • Experience with case management an asset. 

 • Demonstrated ability to produce detailed, high-quality assessment and treatment reports and clinical 

documentation. 

 • Able to communicate and work effectively in an interdisciplinary team environment and willingness to accept 

clinical direction. 

 • Familiarity with the Ontario Mental Health Act and Mental Health reform. 

 • Proficiency in assessment, psychotherapy, and psychosocial counselling techniques for working with 

individuals, groups, and communities. 

 • Demonstrated skill and competence in establishing supportive, trusting relationships with persons living with 

mental health and addictions issues. 

 • Familiarity with signs and symptoms of trauma, able to respond appropriately in making referrals and helping 

clients manage these symptoms. 

 • Experience working with clients with a concurrent disorder and those living in unstable housing situations or 

homelessness.

From hosting workshops to producing educational content, we are committed to strengthening the voice of families in long-term care.To help make this possible, we are seeking an Intake Coordinator (English/French Bilingual).The Role:As the Intake Coordinator, you will be the first point of contact for individuals seeking support from Family Councils Ontario. You will assess client needs, coordinate service referrals, and ensure timely, compassionate, and culturally sensitive responses.Key responsibilities include:<ul><li>Responding to phone and email inquiries and conducting intake interviews</li><li>Assessing clients’ eligibility and needs to connect them to FCO services</li><li>Scheduling consultations, workshops, and presentations</li><li>Maintaining accurate client records using FCO systems (HubSpot CRM)</li><li>Tracking intake metrics and client satisfaction feedback</li><li>Collaborating with the Outreach, Education, and Communications teams</li><li>Recommending process improvements and participating in team meetings</li></ul>Key Qualifications:The ideal candidate will have post-secondary education in social work, community development, or a related field, and at least two years of experience in client intake or service navigation. You should have excellent interpersonal, organizational, and communication skills, as well as a compassionate and client-focused approach.Proficiency in both English and French (written and oral) is required. Experience with CRM systems and the long-term care or nonprofit sector is an asset. A strong understanding of cultural competence and the ability to work independently in a remote environment are essential.Position Details:Salary is: $35,000 annuallyBenefits: Participation in the OPTrust Select pension plan (3% matched contributions) is optional.
