Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a dynamic, community-based 
health care organization serving mid-west downtown Toronto. Offering a broad range of services, including 
primary health care, dental care, harm reduction, health promotion, counselling, and community development

General Responsibilities: The Physician provides comprehensive, primary health care which emphasizes 
accessibility, health promotion, illness prevention and continuity of care for our clients, including families, children, 
refugees, those experiencing homelessness, and other individuals who occupy our large urban catchment. The 
physician functions both independently and as part of a multi-disciplinary team. The work will be based at the 
Queen West site. 

Reporting Manager: Manager, Primary Care 

Specific responsibilities include, but are not limited to: 
 

ï· Providing primary care directly, emphasizing accessibility, health promotion, illness prevention and 
continuity of care. 

ï· Providing primary care indirectly by working interactively with nurse practitioners, providing consultation to 
nurses or other staff, and by assisting in development of medical or clinical protocols. 

ï· Providing on-call coverage on a rotating shared basis with other physicians to ensure that medical care 
can be accessed 24 hours per day throughout the entire year. 

ï· Participating in quality improvement activities 
ï· Acting as a preceptor to medical students and residents and participating in the learning of students from 

other disciplines. 
ï· Providing leadership in the general organization and operation of the clinic’s medical activities. 

 
Qualifications: 

ï· Member in good standing of the College of Physicians and Surgeons of Ontario and the College of Family 
Physicians of Canada.

ï· Experience working with people who have history of substance use and /or mental health issues.

Reporting to the HART Hub Manager, the Chiropodist will participate in larger HART Hub partnerships led by the 
Centre, which may include, but not limited to CAMH, LOFT, and The Neighbourhood Group. The Chiropodist will 
support clients and share knowledge. 
 
General Responsibilities: 
 

 • Conducts assessment and treatment of foot conditions, including but not limited to diabetic foot care, nail 
care, biomechanical assessments, custom orthotic casting and dispensing, wound care, measuring for 
compression stockings, and conducting procedures such as PNA, TNA and soft tissue lesion excisions 

 • Works closely with staff to provide foot care within a broader multi-disciplinary team; works particularly 
closely with the diabetes nurse and diabetes dietitian 

 • Builds trust and rapport with clients to promote effective service use 

 • Provides health education, harm reduction education, and advocacy to HART Hub clients 

 • May provide services at designated outreach locations in the community and in the mobile health unit 

 • Maintains client/patient files through PQWCHC’s EMR, encounter information, correspondence, clinical 
protocols, and other data to meet the requirements of the funders, the clinic and regulatory bodies 

 • May act as supervisor/consultant to health care students and participates in the learning of students from 

other disciplines 

 • Participates in the design, implementation and evaluation of research studies in consultation with the 

Management team and other staff 


Qualifications: 

1. Graduate of a recognized educational institution for Podiatry/Chiropody; certificate of practice in good 
standing from the College of Chiropodists of Ontario to work to full scope, including injection and 
prescription privileges 

2. Demonstrated ability to fit for orthotics 
3. Experience in community-based practice and working with marginalized populations.

The 90 Dunn Avenue Housing Program works in collaboration with the University Health Network’s Social Medicine 
team to support people who are experiencing chronic homelessness who have been high users of Emergency 
Departments, with some focus on people with disabilities and Indigenous Peoples; to access and maintain alternative 
supported housing. This program aims to provide a safe space for individuals experiencing deep poverty with a history of 
chronic homelessness and often the long-term effects of trauma and substance use. Dunn Housing provides housing 
with supports, including case management, on site 24 hours per day, 7 days per week. The program serves tenants 
through a harm reduction, eviction prevention and community engagement framework to reduce their isolation and 
build community through engaging programs and activities.

What You Will Do: 
The Weekend Housing Support Worker will: 

ï· Provide welcoming, friendly and professional front desk support for the tenants at 90 Dunn Avenue 
ï· Support the program by answering phones, fielding tenant questions, taking work orders, assisting with 

medication, giving notices and liaising with other workers and professionals 
ï· Utilize community development, facilitative management, and conflict resolution approaches to facilitate 

supportive, co-operative living and support tenants to develop skills to live cooperatively in shared 
accommodation. 

ï· Facilitate monthly tenant meetings, plan agenda, take minutes and share to all tenants 
ï· Complete annual community needs assessments and create programming based on the communities needs 
ï· Conduct monthly check ins and Housing Assessment tools for tenants who are not assigned to Life Skills or 

Eviction Prevention Case Managers. 
ï· Make appropriate referrals to other supports and resources as required. 


ï· Facilitate and participate in case conferences and meetings with other workers who also provide support for 
the tenants. 

ï· Facilitate quarterly apartment meetings, introductions and conflict resolution processes. Work with 
individual tenants to participate in decisions regarding housing. Facilitate, encourage and promote tenant 
participation in various committees, activities and events. Actively work with tenants to address safety and 
security. 

ï· Educate tenants about their tenancy responsibilities and their on-going responsibilities to maintain their 
housing. Know and be conversant in the Residential Tenancies Act, and in particular, areas on non-payment 
of rent and non-compliance of the tenancy agreement. 

ï· Identify, intervene and deescalate crisis situations and work with tenants to develop crisis management 
skills 

ï· Create plans to assist with any housing related issues such as; behaviour, social isolation, mental health 
care, substance use and hygiene issues by encouraging and supporting tenants to problem solve and to 
access community resources 

ï· Work from trauma informed, community development, harm reduction, client centered and collaborative 
approaches 

ï· Work as part of the Housing Team 
ï· Work in collaboration with University Health Network’s clinical and health navigation teams to support 

tenants to achieve their health and housing related goals 
ï· This position requires respect for, and expertise working with, tenants who are facing mental health issues, 

addictions, abuse and social isolation and who come from racially and ethnically diverse backgrounds.
