Duration: Permanent, full-time 

 
Position Summary: 

 
We are seeking a full-time Health Care Coordinator. Working within a team context, the Health Care 
Coordinator ensures the health and wellbeing of members with intellectual disabilities (Core Members) 
through education, supervision and collaboration around health and medical issues. They also 
participate in planning for appropriate support and services to meet the ever-changing needs of Core 
Members and the assistants who support them. The person in this role is accountable to the Community 
Leader/Executive Director. 

 
Major Responsibilities: 


ï· Work with home and program teams to ensure appropriate care, planning and consistency around 
the medical needs of Core Members; 

ï· Ensure that assistants receive orientation, education and supervision on medication 
administration and management, medication charts, day to day medical procedures, medical 
equipment use, controlled acts, matters of good health, and managing medical conditions; 

ï· Work in collaboration with medical professionals, agency support coordinators, pharmacies, 
L’Arche Ottawa personnel, Core Members and their families around health and medical concerns; 

ï· Ensure individual Behavior Support Plans (BSP) are implemented and kept updated including 
annual signing off. Interface with BSP committee. Records protocols around PRNs within the 
community and for day programs, when appropriate; 

ï· Ensure that health care directions and follow up appointments are carried out. Ensures the 
medical needs of each Core Member are addressed including proper medication storage, 
dispensing, recording and knowledge of each medication in use. Ensure that specialist 
appointments are well prepared for, and appropriate accompaniment is provided for these 
appointments;

ï· Document all medical contacts, concerns, appointments, and long-term care applications. Ensure 
that all health consultation records and binders are accurately maintained and up to date, with 
appropriate medical information in case of emergencies; 

ï· Participate to the Health and Safety Committee of the community, staying up to date for all 
directives and implementation of the compliance rules related to health. 

ï· Participate to professional networks about intellectual disability. 

ï· Meet regularly with the Homes Coordinator for updates, sharing information and planning 
together. Be accountable and keep Leadership informed on a regular basis. 

ï· Carrying the emergency phone in rotation basis with other coordinators 

ï· Any other tasks or activities according to the needs of the organization, and determined by the 
executive director. 


The ideal candidate will possess the following: 
 

ï· Registered Nurse or registered practical nursing certification preferred, or significant health care 

experience and aptitude for medical information; 

ï· Bilingual (English and French); 

ï· Demonstrated experience and passion for supporting individuals with intellectual disabilities and 

individuals with complex behavioural support needs; 

ï· Demonstrated ability to fully implement and monitor behavioural support plans and supporting 
documents; 

ï· Strong interpersonal skills especially communication and advocacy; 

ï· Ability to be flexible and manage the stress of multi-faceted responsibilities and management; 

ï· Ability to remain calm, focused and clear in times of crisis; 

ï· Strong organizational skills and computer capacities. Organized and able to set priorities and 

follow-through on commitments in timely fashion; 

ï· Collaborative, excellent communication skills, including the ability to give direction, report 
information and model effective communication techniques/strategies with the Core Members in 
the home; 

ï· Ability to effectively manage multiple tasks, delegate and supervise; 

ï· Flexible in terms of approach and time; 

ï· Full G Driver’s License and a minimum of 3 years insured driving experience with insurance 

records; 

ï· Maintain automobile insurance with third party liability coverage of a minimum of $2,000,000, and 

coverage for transportation of Core Members without limitation. 


Working Conditions and Physical environment: 
 

 • Will need to work in an office environment with a number of other people, sometimes sharing 
equipment and space, with possibility to work remotely occasionally. 

 • Will need to lead and participate in several meetings, both group and individual. 
 • Is expected to participate in meals in the homes/programs and occasionally attend team meetings 

of each home/program. 
 • Some travel may be required. 
 • Other specific requirements may be expected. 
 
 

Salary: Based on L’Arche Canada’s Compensation Policy. Hourly rate starting at $30.93, based on 
previous experience, skills, and education. Full time employees will be entitled to the Group Insurance 


benefits including life, dependant life, basic and occupational Accidental Death or Dismemberment 
(AD&amp;D), Extended Health Coverage (EHC), Long Term Disability (LTD) as well L’Arche’s Employee 
Assistant Program (EAP) and the group retirement saving plan (RRSP).

Nous sommes à la recherche d'un coordonnateur des soins de santé à temps plein. Travaillant dans 
un contexte d'équipe, le coordonnateur des soins de santé assure la santé et le bien-être des 
membres ayant une déficience intellectuelle (Membres de cÅur) par l'éducation, la supervision et la 
collaboration concernant les questions médicales et de santé. Il participe également à la planification 
du soutien et de services appropriés pour répondre aux besoins en constante évolution des Membres 
de cÅur et des assistants qui les soutiennent. La personne qui occupe ce poste rapporte au 
responsable de la communauté/directeur général. 
 
Principales responsabilités 
 

ï· Travailler avec les équipes des foyers et des programmes de jour pour assurer des soins 
appropriés, une planification et une cohérence autour des besoins médicaux des Membres de 
cÅur. 

ï· S'assurer que les assistants reçoivent l’orientation, les formations et l’encadrement portant sur 
l'administration et la gestion des médicaments, les fiches de médicaments, les procédures 
médicales quotidiennes, l'utilisation des équipements médicaux, les actes autorisés, la bonne 
gestion du bien-être et des questions de santé des membres de cÅur. 

ï· Travailler en collaboration avec les professionnels de la santé, les coordonnateurs de soutien, les 
pharmacies, le personnel de L'Arche Ottawa, les Membres de cÅur et les membres de leur famille 
en ce qui concerne leur santé et les problèmes médicaux. 

ï· Veiller à la mise en place et à la mise à jour des plans de soutien au comportement PSC, y compris 
leur validation annuelle, et assurer la liaison avec le comité PSC. Enregistrer les protocoles relatifs 
aux PRN dans les foyers et les programmes de jour le cas échéant. 

ï· Veiller à ce que les directives en matière de soins de santé et les rendez-vous de suivi soient 
respectés. Veiller à ce que les besoins médicaux de chaque Membre de cÅur soient pris en

ï· Participer aux réseaux professionnels du secteur des personnes ayant une déficience 
intellectuelle. 

ï· Rencontrer régulièrement la coordonnatrice des foyers pour faire le point, partager des 
informations et effectuer la planification. Rendre compte et tenir la direction régulièrement 
informée. 

ï· Porter le téléphone d’urgence en rotation avec les autres coordinateurs 

ï· Toutes autres tâches pour les besoins de L’Arche Ottawa déterminées par le directeur général 
 
 
Le candidat idéal possède les qualités suivantes 
 

ï· De préférence infirmier diplômé ou infirmier auxiliaire diplômé, ou expérience significative dans le 
domaine des soins de santé et aptitude en matière d'informations médicales; 

ï· Bilingue (anglais et français); 

ï· Expérience démontrée et passion pour le soutien aux personnes ayant une déficience intellectuelle 
et aux personnes ayant des besoins complexes en matière de soutien comportemental; 

ï· Capacité à mettre en Åuvre et à se conformer aux plans de soutien comportemental et autres 
documents connexes; 

ï· Solides compétences interpersonnelles, notamment en matière de communication et de défense 
des intérêts des membres de cÅur; 

ï· Flexibilité et résistance au stress dans le cadre de responsabilités multiples; 

ï· Capacité à rester calme, concentré et lucide pendant les temps de crise; 

ï· Solides compétences organisationnelles et informatiques; Capable d’établir des priorités et d’aller 
de l’avant en respectant ses engagements dans les délais impartis; 

ï· Collaborative, avec d’excellentes compétences en communication pour donner des directives, 
écrire des rapports, et entrer en communication effective avec les membres de cÅur dans les 
foyers; 

ï· Capacité à gérer des taches multiples, à déléguer et à superviser ; 

ï· Flexibilité en termes d’horaires; 

ï· Permis de conduire classe G complet et un minimum de 3 ans d'expérience de conduite avec 
preuve d’assurances pour ces 3 années. 

ï· Maintenir une assurance automobile avec une couverture de responsabilité civile d'un minimum 
de 2 000 000 $, et une couverture sans limitation, pour le transport des Membres de cÅur.

ï· Travailler dans un environnement de bureau partagé, avec possibilité de télétravail 
occasionnel. 

ï· Participer à des réunions en groupe ou individuelles.

Salaire et avantages : 

Taux horaire à partir de 30,93 $, basé sur l’expérience, les compétences et les études, conformément

Health Care Coordinator

Summary Working closely with the CDEPO team, the Diabetes Educator (Nurse) is responsible for the delivery and promotion of the Community Diabetes Education Program of Ottawa, across various locations in the city. Using a self-management approach, the Diabetes Educator provides group and individual education to adults with prediabetes or type 2 diabetes, supports starting them on insulin, assists them with titration, and supports with overall chronic disease management.&nbsp; The Diabetes Educator works collaboratively with community physicians and endocrinologists especially around insulin initiation and medication management. In addition, the Diabetes Educator collaborates with other Centretown Community Health Centre (CCHC) teams where appropriate and refers internally and externally as needed.
