The Registered Nurse will work with a multi-disciplinary health care team in the provision of individual client falls assessment, intervention, education and outreach based on best practices.&nbsp;Job Specific Responsibilities&nbsp;<ul><li>Using a client centered approach, the Registered Nurse Provides individual assessment, education, and outreach to seniors.</li><li>Within the scope of a registered nurse including appropriate assessments and counseling, screening, referral, education, and follow-up and other clinical activities consistent with the College of Nurses of Ontario standards of nursing practice and in collaboration with the WEIFPP team.</li><li>Keeps complete, accurate and timely records of client care.</li><li>Works autonomously in community setting through outreach and home visits and will assess, manage and document clinical risk.</li><li>Acts as a health resource and provides appropriate support and consultation to staff other health care team staff.</li><li>Participates in the preparation of new materials based on best practices for client education.</li><li>Participates in regular chart reviews and case conferences.</li><li>Provides health promotion activities as appropriate to the position.</li><li>In consultation with the Program Manager and health care team, develop policies, procedures and protocols for client management and for quality care.</li><li>Participates actively in information sharing within the program, team and other partner agencies.</li><li>Assists in the collection and analysis of relevant data for administration, planning and evaluation.</li><li>Works with internal and external service providers as well as relevant community groups.</li><li>Participates in the development of primary care and health education programs based on client and community identified health needs.</li><li>Participates in community outreach activities related to falls prevention.</li><li>Performs other tasks related to the Integrated Falls Program as assigned by Supervisor.</li></ul>&nbsp;Qualifications&nbsp;<ul><li>Baccalaureate Degree in Nursing. </li><li>Current (and in good standing) Registration with the College of Nurses of Ontario. </li><li>Minimum of 2 years of nursing practice in a community setting&nbsp;&nbsp; </li><li>Professional nursing experience working with seniors, low-income families and multicultural communities.</li><li>Self-motivated, organized, and able to work independently while collaborating effectively with a team</li><li>Demonstrated ability to work in a multi-disciplinary team environment.</li><li>English essential; Bilingualism and other languages an asset.</li><li>Administrative Skills: completing reports and maintaining client records; working in a computerized (Windows) environment.</li><li>Valid driver’s license and current, adequate vehicle insurance, use of a vehicle.</li></ul>
