Position Summary 

 
Funded by the Ontario Black Youth Action Plan, the Student Family Advocate (SFA) will design, 

develop and deliver highly targeted, community-based and culturally relevant advocacy supports 

tailored to the needs of Black students and their families. The SFA will provide advocacy supports and 

1 on 1 Case Management, social programming, and community education sessions benefitting Black 

youth ages 13-25 and their families, including newcomers and those experiencing poverty, in Toronto’s 

York South-Weston and Oakwood-Vaughan communities. The long-term goals of the SFA program are 

to: 


1. Create conditions for Black student success: Black students, families and communities have 
positive and collaborative relationships with schools/education systems. Schools/education 
systems are informed by and responsive to the needs of Black students, families and 
communities, and identify and address systemic barriers to their achievement, equity and 
well-being. 

2. Address outcomes disparities: Black students are better supported and see increases in 
educational achievement and attainment/retention, including successful transitions. 

 
This will be done by carrying out the following objectives: improving the experience of Black high 
school aged youth, and Black youth transitioning into postsecondary education and connecting youth 
to specialized services (including employment support, legal support, and educational support).

INDIGENOUS PEER LEADER – 3 CONTRACT POSITIONS 

Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a dynamic, community-based health 
care organization serving mid-west downtown Toronto. Offering a broad range of services, including primary health care, 
dental care, harm reduction, health promotion, counselling, and community development programming, we are also 
committed to serving people who face barriers to health care, and we strive to reduce these barriers. PQWCHC is a multi-
sited organization. 
 
Programme Overview: The Niiwin Wendaanimak (Four Winds) Aboriginal Wellness Program supports people who 

identify as Aboriginal with a focus to support those who are homeless, have experienced homelessness and/or under-

housed. The program is housed at the Parkdale Queen West Community Health Centre, Queen West site and provides 

culturally specific programs including access to healing ceremonies, health education, case management supports, 

access to primary care and harm reduction supports. 

General Responsibilities: 

 • Assists with programme shopping, meal preparation and budget adherence. 

 • Assists in programme activities and in general programme agenda e.g. supporting crafts, games, drumming. 

 • Assists with the traditional aspects of the program by being a helper for the Elder. 

 • Attends meetings and skill-building workshops. 

 • Helps in planning and preparation of new programme activities. 

 • Supports participants in the programme and supports programme goals 


Qualifications &amp; Experience: 

 • Have experience of working within Indigenous communities from a philosophy of non-interference and harm 

reduction. 

 • Willingness to work in a harm reduction program. 

 • Gets along with people, does not judge and willing to learn. 

 • Volunteering or previous work experience is an asset. 


This position is for up to 6 months with the possibility of extension. There is a weekly peer rotation – Peers will alternate 

work every Monday 12-3 &amp; Tuesday from 9:00am-1:30 pm at our Parkdale site, 1229 Queen St West. Thursdays 11-3 or 

12-3 with supervision scheduled weekly for each peer &amp; Fridays 9-2 pm at 168 Bathurst Site. Workshops &amp; planning 

meetings are held throughout the year that you will be paid to attend.

General Responsibilities: We are seeking a Chiropodist works to enhance the foot health of homeless and 
under housed individuals, as well as individuals with diabetes, and seniors, by providing direct foot care, surgical 
procedures, and health promotion services as part of the primary care team and programs. 

Reporting Manager: Manager, Primary Care Services 

General Responsibilities 

1. Assessment and treatment of foot conditions, including but not limited to diabetic foot care, nail care, 
biomechanical assessments, custom orthotic casting and dispensing, wound care, measuring for 
compression stockings, and procedures such as PNA/TNA/soft tissue lesion excisions. 

2. Instrument reprocessing. 
3. Works closely with staff to provide foot care within a broader multi-disciplinary team. 
4. May provide services at designated outreach locations in the community. 
5. Maintains required records, reports and statistics including supplies inventory. 
 

Qualifications: 

1. Graduate of a recognized college of Podiatry/Chiropody with a certificate of practice by the College of 
Chiropodists in Ontario to work to full scope, including injection and prescription privileges. 

2. Demonstrated ability to fit for orthotics. 
3. Experience in community-based practice and working with marginalized populations. 
4. Ability to work as part of an interdisciplinary team. 
5. Excellent communication and problem-solving skills. 
6. Ability to work in a flexible manner which will ensure client access to service. 
7. Good organizational and time management skills. 
8. Ability to speak a second language is an asset. 
 

Compensation: $69,599 to $81,343 per annum, plus HOOPP (Healthcare of Ontario Pension Plan) option.
