Davenport-Perth Neighbourhood and Community Health Centre (DPNCHC) provides programs and services to diverse participants who are marginalized due to socioeconomic barriers. As a multiservice community agency, we prioritize community development and health promotion. Our Health Centre is a dynamic and complex practice where a multidisciplinary team of nurses, nurse practitioners, counsellor/therapists, a dietitian, and physiotherapists collaborate to deliver holistic care.

Qualifications:

• Current registration and in good standing with the College of Physicians and Surgeons of Ontario
• CCFP certification preferred
• Excellent clinical and interpersonal skills
• Experience working in a community-based organization with diverse cultures
• Strong commitment to community-based health-care delivery
• Sensitivity and understanding when working with high-need populations
• Ability to work collaboratively as part of an interdisciplinary health care team
• Ability to speak a second language is an asset
• Some evening hours will be required.

Contract dates: November 30, 2026 to May 28, 2027.

Salary Range: $307,184-$331,973 per annum, pro-rated to FTE, plus statutory benefits and Healthcare of Ontario Pension Plan (HOOPP). This position is not part of the bargaining unit.
