TAIBU Community Health Centre (“the Centre”), located in Malvern, Scarborough, provides primary health care and related services for Black populations across the GTA and local residents. Guided by intersectional, equity-based, and culturally affirming practices, TAIBU promotes holistic wellness, health education, and prevention.

ABOUT THE ROLE

TAIBU Community Health Centre is seeking a LEARN Program Facilitator to support two youth-focused programs: the LEARN Mentorship Program and Step It Up Girls. The position combines direct program facilitation, participant and family engagement, mentorship support, program documentation, evaluation and routine administration. Grounded in TAIBU’s Model of Black Health and Wellbeing, the role supports culturally responsive, strengths-based programming for Black children, youth and families. The 24-hour work week is generally structured as 14 hours for LEARN, 7 hours for Step It Up Girls, and 3 hours of dedicated in-office administrative time.

KEY RESPONSIBILITIES

  • Lead weekly LEARN after-school programming, including educational, mentorship, social and culturally responsive activities.
  • Lead the weekly Step It Up Girls after-school program, with a focus on wellbeing, confidence, social connection and leadership development.
  • Build supportive relationships with participants and maintain appropriate communication with parents and caregivers.
  • Work collaboratively with LEARN mentors and with the Enhanced Youth Outreach Worker to support participant needs and engagement.
  • Support appropriate cross-referral between Step It Up Girls and the LEARN Mentorship Program.
  • Work with TAIBU’s dietitian and program teams to plan nutritious, culturally relevant, child- and youth-friendly, fiscally responsible meals and snacks for both programs.
  • Maintain accurate and timely participant encounters, attendance, program activities and statistics in TAIBU’s electronic medical record system.
  • Support participant feedback, evaluation, reporting and continuous program improvement.
  • Complete routine program administration and financial processes, including program records, petty cash, receipts and expense documentation.
  • Use dedicated in-office administrative time for documentation, planning, reporting and follow up.
  • Support field trips, special activities, outreach and community events, and connect participant concerns or emerging needs to appropriate TAIBU staff or services.

Summer Logistics Support — July and August

During July and August, when regular LEARN after-school programming is paused, the Facilitator provides logistics support to TAIBU’s Canada Summer Jobs program and summer youth activities. This includes maintaining activity and event schedules, gathering logistical requirements from program leads, communicating event details to summer staff, and supporting transportation and scheduling follow-up. The Manager, Community Programs leads Canada Summer Jobs recruitment, contracts, orientation, training and staff supervision.

QUALIFICATIONS

  • Diploma or degree in Child and Youth Care, Social Services, Community Work, Education, Social Work, Health Promotion or a related field, or an equivalent combination of education and relevant experience.
  • Minimum one year of experience working directly with children and/or youth.
  • Experience facilitating youth, educational, after-school or mentorship programming.
  • Experience engaging Black children, youth, families and communities.
  • Understanding of anti-Black racism and systemic barriers affecting Black children, youth and families.
  • Experience with program documentation, data collection, evaluation or reporting.
  • Strong facilitation, communication, organizational and relationship-building skills.
  • Strong attention to detail and comfort using electronic documentation and standard computer systems.
  • Ability to work independently and collaboratively, maintain professional boundaries and confidentiality, and work occasional evenings and weekends.
