Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a dynamic, community-based 
health care organization serving mid-west downtown Toronto. Offering a broad range of services, including 
primary health care, dental care, harm reduction, health promotion, counselling, and community development 
programming, we are also committed to serving people who face barriers to health care, and we strive to reduce

General Responsibilities: We are currently seeking a Registered Nurse to provide a range of client-centered 
health care services to individuals, families and communities at all life stages. The RN will work to their full scope 
of practice as a member of the interprofessional team. They will work within a harm reduction framework to 
provide a broad spectrum of health promotion, disease prevention, chronic disease management, education, 
treatment and supportive care. 
 
Reporting Manager: Manager, Primary Care 
 
Specific Responsibilities: 
 • Triage incoming patient calls. Assess patient health care needs, discuss test results and respond to questions 

pertaining to health promotion and disease prevention. Redirect calls to booking when necessary 
 • Maintain the medical record alert system to recall patients for health promotion and disease prevention follow-up 

such as pap smears, mammograms, FIT testing, and hypertension testing 
 • Administer and record immunizations and allergy injections and assist in vaccine management 
 • Assist in Infection Prevention &amp; Control (e.g., instrument reprocessing) 
 • Perform clinical procedures such as pap tests, suture removal, skin and wound dressings, TB testing, wart 

treatment, and ear-syringing 
 • Perform blood pressure checks, assessing response to medications, educating patients and promoting health 

through blood pressure management 
 • Participate in home visits and outreach nursing, as required 
 • Work collaboratively with the health care team in the development, implementation and monitoring of chronic 

disease management programs around key health issues 
 • Engage in collaborative community relationships to facilitate patient access to services. 
 • Assist in the training and development of new staff and nursing students. 
 
Qualifications: 
 • Bachelor of Science in Nursing degree from a recognized program 
 • Current registration as a Registered Nurse with the College of Nurses of Ontario (CNO) 
 • 3 or more years of experience in a community setting or combination of community, hospital or public health 

settings preferred 
 • Experience in primary care or street/outreach nursing an asset 
 • Experience in emergency response an asset 
 • Experience in program development, implementation monitoring and evaluation an asset

The World Federation of Hemophilia (WFH - <a href="http://www.wfh.org" target="_blank">http://www.wfh.org</a>) is an international not-for-profit 
organization that provides global leadership to improve and sustain care for people with inherited 
bleeding disorders, including hemophilia, von Willebrand disease, rare factor deficiencies, and inherited 
platelet disorders. Established in 1963, the WFH works with member organizations in 151 countries as 
well as through a network of international volunteers and healthcare providers to achieve our vision of 
"Treatment for All" around the world.

The WFH is looking for an experienced, results-oriented REGIONAL MANAGER with superior 
project / health program management abilities and interpersonal skills, to represent the WFH 
and coordinate the implementation of its country and health care development programs and 
activities in South America (12 countries). He/she will further act as conduit between WFH 
headquarters and national member organizations (NMO) and bleeding disorders community 
within the region.

Principal Responsibilities - 

Reporting to the Head, Global Development at the WFH headquarters, the Regional Manager 
– South America will be responsible for the development, management, implementation, and 
delivery of a portfolio of healthcare programs at the country and regional level and for 
overseeing the implementation and follow up of WFH-led world healthcare development 
programs and activities for designated countries within the region (Argentina, Bolivia, Brazil, 
Chile, Colombia, Guyana, Ecuador, Paraguay, Peru, Suriname, Uruguay and Venezuela).

As the main representative for the WFH within the region, the Regional Manager will carry out 
the following tasks: 

 • Represent the WFH within the region and assist the organization in defining strategic 
objectives and work plans for the region 

 • Provide guidance and support to WFH national member organizations (NMOs) in South 
America and maintain the collaboration national and regional stakeholders (HCPs, 
governments, other organizations and industry) 

 • Ensure effective development, planning, coordination, monitoring, reporting, and 
evaluation of the following WFH bleeding disorder programs in the region, including: 

â¢ Country development programs (Country Programs, Cornerstone Initiative, 
PACT) 

â¢ Regional workshops, conferences and other regional activities targeting 
people with bleeding disorders and healthcare professionals 

â¢ Medical and organizational and youth twinning partnerships 

â¢ Contribute to raising awareness and to the implementation of other WFH-led 
programs (example: humanitarian aid, global training programs &amp; initiatives, 
data collection) offered to designated regions. 

 • Facilitate and support the work of key volunteers and WFH committees; respond to 
queries and correspondence in regard to programs and activities 

 • Conduct country visits with expert volunteers to meet with the NMOs, senior 
government officials, as well as hemophilia treatment centers, hospitals, blood banks 
and other organizations as required 

 • Ensure sound administrative and financial control of budgets and planned program 
activities spending in the region, including timely production of progress reports and 
other relevant documents 

 • Document visits (written reports &amp; visuals) and author articles for WFH online news and 
social media, on current programs and activities in the region. 

Carry out other tasks relevant to the position as required by the Head, Global Development, 
the Director, Global Development, or the CEO as needed.

QUALIFICATIONS and REQUIREMENTS – 

 • Bachelor’s degree in public health, international development or relations, political 
studies, social sciences, or other relevant discipline 

 • Six to nine (6-9) years of relevant work experience with a world health development 
organization – notably with bleeding disorders - that includes at least five years 
(essential) of proven track record in country project/ health program management, and 
monitoring &amp; evaluation of health programs in South American countries (Argentina, 
Bolivia, Brazil, Chile, Colombia, Guyana, Ecuador, Paraguay, Peru, Suriname, Uruguay, 
Venezuela); experience working in a similar regional role – an asset 

 • Solid understanding of socio-economic conditions and world health (example: 
noncommunicable diseases, or rare diseases) developmental issues in the stated region 

 • Excellent diplomatic and cross-cultural skills, and demonstrated experience in working 
with people from diverse cultures and countries; superior abilities to build/maintain

CALIFICACIONES y REQUISITOS – 

 • Licenciatura en salud pública, desarrollo o relaciones internacionales, estudios 
políticos, ciencias sociales u otra disciplina relevante. 

 • De seis a nueve (6-9) años de experiencia laboral relevante en una organización 
mundial de desarrollo sanitario, especialmente en el ámbito de los trastornos de la 
coagulación, que incluya al menos cinco años (imprescindible) de experiencia 
demostrable en la gestión de proyectos nacionales/programas sanitarios y en el 
seguimiento y la evaluación de programas sanitarios en países sudamericanos 
(Argentina, Bolivia, Brasil, Chile, Colombia, Guyana, Ecuador, Paraguay, Perú, Surinam, 
Uruguay y Venezuela). Experiencia trabajando en un puesto regional similar: una 
ventaja. 

 • Sólida comprensión de las condiciones socioeconómicas y las cuestiones de desarrollo

The role of OPSW includes in situ harm reduction support for shelter residents self-isolating as per directions from 
Toronto Public Health, to prevent COVID transmission within the shelter system. OPSW’s will mobilize COVID 
response should the need to uplift a new isolation site(s) emerge and will abide and enforce strict infection control 
measures during overdose response. This position may also require flexibility to work late evenings, overnights, 
and weekends, as required by the covid isolation support function of the job. 
 
General Responsibilities: 
 
The OPSW works in tandem with Client Support Workers (CSW) and Harm Reduction Coordinators to assess risk 
factors in sheltering sites for development of effective overdose response strategies. The OPSW leads response 
to overdoses and facilitates drills and simulations for the LAUNCH team, shelter staff and community members. 
The OPSW is expected to facilitate prevention and response work in controlled and uncontrolled settings and is 
responsible for completing quality assurance on all equipment and provides safer injection education and safety 
planning to clients accessing LAUNCH services. The OPSW is responsible for capturing client encounters within 
an agency communication database. 
 
Qualifications: 

1. Undergraduate in a relevant discipline from a recognized university or some post-secondary education 
combined with significant experience in health promotion and/or community development or an equivalent

The HART Hub Chiropodist is part of the HART HUB services which will work closely with the mental health, 
substance use care (continuum of treatment, recovery, harm reduction), and health promotion teams to ensure 
cross-team and cross-site collaboration. The Chiropodist will work with the team to contribute to the growth and 
development of the HART Hub program which will include the development of processes and pathways for HART 
Hub clients. 
 
Reporting to the HART Hub Manager, the Chiropodist will participate in larger HART Hub partnerships led by the 
Centre, which may include, but not limited to CAMH, LOFT, and The Neighbourhood Group. The Chiropodist will 
support clients and share knowledge. 
 
General Responsibilities: 
 

 • Conducts assessment and treatment of foot conditions, including but not limited to diabetic foot care, nail 
care, biomechanical assessments, custom orthotic casting and dispensing, wound care, measuring for 
compression stockings, and conducting procedures such as PNA, TNA and soft tissue lesion excisions 

 • Works closely with staff to provide foot care within a broader multi-disciplinary team; works particularly 
closely with the diabetes nurse and diabetes dietitian 

 • Builds trust and rapport with clients to promote effective service use 

 • Provides health education, harm reduction education, and advocacy to HART Hub clients 

 • May provide services at designated outreach locations in the community and in the mobile health unit 

 • Maintains client/patient files through PQWCHC’s EMR, encounter information, correspondence, clinical 
protocols, and other data to meet the requirements of the funders, the clinic and regulatory bodies 

 • May act as supervisor/consultant to health care students and participates in the learning of students from 

other disciplines 

 • Participates in the design, implementation and evaluation of research studies in consultation with the 

Management team and other staff 


Qualifications: 

1. Graduate of a recognized educational institution for Podiatry/Chiropody; certificate of practice in good 
standing from the College of Chiropodists of Ontario to work to full scope, including injection and 
prescription privileges

Striving for excellence in client-centred care and education, you will work as part of the Mobile Covid and Harm 

Reduction Outreach Services (LAUNCH) team. In the role of a LAUNCH Overdose Prevention Support Worker, 

you will provide health information and support on safer injection and consumption, monitor clients for adverse 

reactions, assess, respond to, and support overdose events, and assist in the initiation of emergency responses, 

as necessary. As a member of the LAUNCH team you will assist in educating team members in areas relating to 

overdose response, infection prevention and safer drug consumption for purposes of increasing team capacity to 

support the needs of people who use drugs (PWUD) in sheltering sites. 


The role of OPSW includes in situ harm reduction support for shelter residents self-isolating as per directions from 

Toronto Public Health, to prevent COVID transmission within the shelter system or any further emerging public 

health needs. This position may also require flexibility to work late evenings, overnights, and weekends across 

various shelter and respite locations, to meet operational needs. 

 • Evening Shifts: 4:00 PM to 12:00 AM (midnight) 

 • Overnight Shifts: 12:00 AM (midnight) to 8:00 AM 

General Responsibilities: 


The OPSW works in tandem with Client Support Workers (CSW) and Harm Reduction Coordinators and our 

partners to assess risk factors in sheltering sites for development of effective overdose response strategies. The 

OPSW leads response to overdoses and facilitates drills and simulations for the LAUNCH team, shelter staff and 

community members. The OPSW is expected to facilitate prevention and response work in controlled and 

uncontrolled settings and is responsible for completing quality assurance on all equipment and provides safer 

injection education and safety planning to clients accessing LAUNCH services. The OPSW is responsible for 

capturing client encounters within an agency communication database. 


Qualifications: 

1. Undergraduate in a relevant discipline from a recognized university or some post-secondary education 
combined with significant experience in health promotion and/or community development or an equivalent 
of work/lived experience. 

2. 2-4 years working in overdose response capacity with people who use drugs, racialized populations, 
Indigenous peoples, people who engage in sex work, prisoners/ex-prisoners, people with mental health 
challenges, and people who have experienced trauma 

3. 3-5 years of experience in a community setting should include demonstrated knowledge of community 
resources, harm reduction, and supportive counselling 

4. Excellent written and verbal communication skills 
5. Extensive advocacy, crisis intervention and prevention, conflict resolution, and negotiation skills 

6. Ability to travel across the City of Toronto 

7. Ability to develop and implement harm reduction risk analysis assessments and overdose prevention and 

response protocols 

8. Demonstrated ability to work with multi-disciplinary teams. 

9. Experience with overdose prevention and responding to overdose emergencies 

10. Proficiency in a language other than English, as well as personal/lived experience of drug use, 

homelessness and/or street involvement are assets.
