POSITION SUMMARY
The Healthcare Navigator is responsible for providing direct patient care and comprehensive coordination of care for clients and their families/caregivers. Working within the principles and values of TAIBU Community Health Centre, the Navigator will work to eliminate barriers to timely care and enhance care coordination to the various services needed to manage the client’s health care needs and improve health outcomes. The Navigator will serve as a link between the patient, primary care, allied health and social services. The role will participate in the client intake, assessment, care plan and referral process within the multi-disciplinary clinical team. The Navigator will also be called upon to support the health promotion, chronic disease prevention and advocacy initiatives of the Centre. The Navigator will work within a client-centred and an anti-racist/anti-oppressive framework.
RESPONSIBILITIES
Primary Health Care:
- Act as clinical liaison, resource and navigator for TAIBU CHC clients by providing timely assistance to clients as they navigate the healthcare system.
- Provides direct nursing care for TAIBU CHC patients within nursing scope of practice including prenatal, well baby visits, cancer screening, pediatric and adult immunization etc.
- Participates in the client intake, assessment, triage, care plan and referral process of the Centre for new and existing patients
- Coordinates care plans including planning, scheduling and implementation of procedures for patients identified as needing navigation
- Assist in managing patient referrals to the appropriate health specialties, diagnostic services and community services where indicated (including helping patients book appointments, following-up on referrals and providing reminders etc.)
- Provides education to patients, families and caregivers while assisting and empowering them to engage in self-management of chronic conditions
- Assist with forms completion, system navigation and advocacy for clients as required
- Ensure clients, families and caregivers can access and understand health information; lead and/or help develop client healthy literacy initiatives
- Collaborate with all providers while maintaining appropriate and timely communication with members of the primary care, community and systems change and administrative teams
- Participate in programs aimed at health education and self-management of chronic conditions
- Makes sound decisions based on knowledge and full analysis of the presenting situation or problems
- Participate in quality assurance and quality improvement initiatives that may benefit or improve the service
- Participate in research and prepares reports regarding client care and healthcare access, identifying needs within the community and participating in the development of new programs which address those needs
- Participate on the Centre’s committees, advocacy efforts, case conferences and planning meetings as needed or as assigned
- Other duties as assigned or needed to be successful in achieving the objectives of the role
Administrative Duties
- Share administrative and clinical maintenance tasks with other Centre staff. These may include compiling statistical data, updating records, peer review of client records.
- Maintains appropriate patient records and assures all encounters are entered into the Centre’s EMR
- Vetting, approving, editing and organizing health education materials
- Participate/lead in/ research initiatives
Accountability
- This position reports to the Director Primary Healthcare Services
- The Health Care Navigator is accountable to meet the requirements of any accountability agreement between Ontario Health and TAIBU CHC

- Maintaining up-to-date professional skills and knowledge through formal and informal training
- Acting as an “Ambassador” for TAIBU CHC within and outside the Centre, creating an awareness of TAIBU programs and promoting services offered
- Compliance with all mandated legal/regulatory requirements related to working at TAIBU CHC e.g. Employment Standards Act, Occupational Health and Safety, standards of professional conduct, etc.
- Maintaining awareness of, and compliance with, TAIBU CHC policies and procedures
- Maintaining awareness of, and compliance with, funder requirements, policies and procedures, regulations
- Attending team and staff meetings as required
Qualifications
- University Degree in Health or Social Sciences with a minimum of three years work in community health or social sciences setting
- Registration with the College of Nurses without limitations
- Experience working within local health care system and extensive knowledge of clinical and community resources
- Ability to perform nursing assessments and procedures
- Ability to educate patients and their families regarding disease management
- An understanding, patient individual who is sensitive to people, with a variety of health/social and economic needs
- Enjoys working in a multidisciplinary team environment
- Experience in the use of computers and appropriate software applications
- Enthusiasm and interest in working with TAIBU CHC priority populations
- An interest in developing new skills relative to identified needs of clients
- Bilingual and comfortable to practice nursing in both French and English is an asset
Compensation:
- $64,772 – 78,038 + Competitive benefits
- Expected salary adjustment in the near future – based on Ministry communication
- New recruits at TAIBU CHC will be enrolled in the Healthcare of Ontario Pension Plan (HOOPP)
