About the Position: Journey Facilitators work alongside individuals through a deep-dive discovery that explores identity, motivations, gifts, and goals. To offer guidance to individuals in navigating which of the outreach services offered by posAbilities is the right fit for their future goals. To provide ongoing case management throughout individual service journey. About YOU: • Well-developed interpersonal and verbal/written communication skills&nbsp; • Ability to work independently and as part of a team&nbsp; • Commitment to the Association’s Code of Ethics, Values and Mission&nbsp; • Working knowledge of information technology&nbsp; • Strong administrative skills&nbsp; • Good organization and time management skills&nbsp; • Knowledge of the needs of youth in transition and adults with developmental disabilities&nbsp; • Familiarity of Association and local community resources and servicesYou MUST have the following: • Diploma in Social Work, Social Sciences, Human Services or an equivalent combination of education and experience • Minimum two years of experience working in a highly collaborative environment and with multi-stakeholder groups • Demonstrated ability to work with individuals with diverse and unique communication styles&nbsp; • Class 5 driver’s license, acceptable driving record&nbsp; • Reliable car as required and $3,000,000 insurance liability&nbsp; • Current Emergency 1st Aid and CPR&nbsp; • Current TB test&nbsp; • Criminal Record Check for Children and Vulnerable Adults - clearanceYOUR Typical Responsibilities:Facilitating Deep-dive Discoveries • Facilitates a service journey experience from a person-centered approach, with an emphasis on self-determination, identity, values, and individuality. • Works alongside participants to inspire and motivate them throughout a facilitated self- exploration, contributing to the co-design of their action plans outlining their goals, especially their ability to choose fuller and meaningful lives. • Uses discovery tools to initiate conversations and understanding around service options, providing the participant with information to make informed decisions about the service journey. • Builds the competencies and confidence of participants to self-initiate around the journey inward, including spirituality, philosophy and identity, and to reflect on the sort of life they want to live.Service Coordination&nbsp; • Implements the strategies, principles and frameworks for Explore within the Association.&nbsp; • Provides information on all services offered by the Association to participants and otherstakeholders.&nbsp; • Coordinates, designs, monitors and plans service journeys based on desired individualoutcomes with practice support from the Journey Coordinator.&nbsp; • Responsible for fostering and coordinating communication with participants, their networks,stakeholders, and other Association services. This involves convening regularly throughout participants’ service journeys, including roundtables, introductory meetings, and ongoing updates. • Seeks and mobilizes resources/partnerships/community assets to support the needs of individuals throughout their Explore journey. • Collaborates with participants, their networks, and other Association services to address any issues that arise during their service journey with support from the Journey Coordinator.Case Management • Monitors, adapts and troubleshoots service journeys with practice support from the Journey Coordinator. • Liaises and works alongside professionals &amp; staff who are involved in individuals’ service journeys. • Inputs data in a timely manner and ensures that it is accurate and current including participant information on the database. • Completes all required documentation and reporting.&nbsp; • Participates in meetings with other stakeholders with direction from the Journey Coordinator.&nbsp; • Manages a caseload and monitors the number of participants including the intensity of supportknown for each participant. Informs the Journey Coordinator when there is capacity for additional referrals. • Responsible for creating and prioritizing their daily schedule informed by participant and service needs.Facilitation&nbsp; • Provides opportunities for staff within the Association and other stakeholders tounderstand the frameworks and principles of the Explore model of service in order to facilitate a cohesive network of support. • Facilitates workshops for posAbilities staff on adapted Explore curriculum.&nbsp; • Facilitates monthly workshops for Explore participants to engage in furtherdiscussion around some of the topics or themes present in their service journeys (e.g. self-determination, identity).Participation in development • Customizes discovery tools and activities to respond to individual learning and communication styles.&nbsp; • Contributes to the development of discovery tools for participants, new methods ofpractice, and system improvements.&nbsp; • Contributes to the development of curriculum for Explore participant workshops as wellas the adaptation of curriculum to be used more broadly.&nbsp; • Participates in collaborative meetings with other agencies that have wraparound servicemodels to discuss practice and shared touchpoints.&nbsp; • Identifies potential areas for policy, procedural or structural changes within the servicedelivery system and relays this information to the Journey Coordinator.&nbsp; • Participates in the development, implementation and analysis of service evaluation and impact.&nbsp; • May represent the Association at a professional and community level (e.g. committees) andparticipates in the organization’s special events and marketing.&nbsp; • Collects participant and natural support stories to reflect significant changes in individuals’lives as impacted by service journeys within Explore Additional duties&nbsp; • Participates in all aspects of the Health and Safety Program including safety andemergency procedures, in order to provide a safe environment. Bike the Night is back for its sixth edition on Saturday, September 13th! This bright, high-energy 10km seawall ride and fundraiser will kick off at David Lam Park with pre-ride celebrations including an E-Bike Show, live music, family-friendly activities, and opportunities to connect with our community partners!&nbsp; To ensure a safe and enjoyable experience for all, we are seeking trained First Aid Volunteers to be on-site during the event. You’ll play a key role in promoting rider well-being, responding to minor injuries or incidents, and supporting our team in ensuring participants feel safe throughout the event. And to lobby with alternative and mass media in Peru and with foreign press correspondents in Peru. You will prepare press releases, take or obtain photographs and videos, write articles and disseminate them on the department's social networks and different media, if necessary.Translate press releases, articles, communiqués and other communication products into English for dissemination to international media and networks of activists, NGOs and other solidarity groups and IDL allies.Broadcast press conferences, forums and other events on IDL's Facebook Live and/or other virtual broadcasting channels.
