About Across Boundaries: 

Across Boundaries is a mental health center that provides a range of mental health and addictions support and 

services to individuals from racialized and Black communities in the Greater Toronto Area. We have a holistic approach 

to mental health care and operate within an anti-racism/anti-oppression and resisting anti-Black racism frameworks. 

These frameworks focus on providing opportunities to respond to systemic barriers, advocate for the inclusion of 

traditionally and historically silenced voices at decision-making tables, and strive for change at individual, 

organizational and systemic levels. 

Job Summary: 

The Social Worker will report directly to the Program Manager. They will work collaboratively with various teams to 
provide prompt and appropriate access to services for racialized individuals from the community who are experiencing 
mental health and addictions issues. The worker will provide both intensive and non-intensive services to clients based 
on their needs and will have a caseload of up to 20 to 25 program participants. The Social Worker will be expected to 
work directly in the community using the agency van for outreach and support to community members. 
 
Normal Hours for full-time employees consist of five (5) shifts or eight (8) hours per day, Monday to Friday, and Saturdays 
on a rotating basis, inclusive of a thirty (30) minutes unpaid lunch break (entitled under the ESA) and an additional two 
(fifteen (15) minute unpaid rest breaks. Normal hours of work will generally be scheduled between 8:00 am and 8:00 
pm. Employees may be requested, but not required, to work some Sundays. 

Qualifications &amp; Responsibilities: 
 

 • Master’s degree in social work from a recognized university or equivalent. 

 • Knowledge of holistic, anti-racism/anti-oppression and resisting anti-Black racism frameworks as guiding 

principles for assessing and developing treatment plans for clients. 

 • Supervised clinical training. 

 • Practical experience in the use of Dialectical Behaviour Therapy (DBT) and Cognitive Behavioral Therapy (CBT) 

and able to provide psychotherapeutic assessments and structured counseling to individuals presenting with a 

wide range of mental health conditions. 

 • Clinical competence to provide structured counseling in the areas of trauma, depression, anxiety, grief/loss, 

and/or addictions. 

 • Experience with case management an asset. 

 • Demonstrated ability to produce detailed, high-quality assessment and treatment reports and clinical 

documentation. 

 • Able to communicate and work effectively in an interdisciplinary team environment and willingness to accept 

clinical direction. 

 • Familiarity with the Ontario Mental Health Act and Mental Health reform. 

 • Proficiency in assessment, psychotherapy, and psychosocial counselling techniques for working with 

individuals, groups, and communities. 

 • Demonstrated skill and competence in establishing supportive, trusting relationships with persons living with 

mental health and addictions issues. 

 • Familiarity with signs and symptoms of trauma, able to respond appropriately in making referrals and helping 

clients manage these symptoms. 

 • Experience working with clients with a concurrent disorder and those living in unstable housing situations or 

homelessness. 

 • Demonstrated ability to work independently, cooperatively, and constructively within a multidisciplinary team 

environment.

services to individuals from racialized communities who are experiencing mental health problems and substance use 

challenges. The workers will provide both intensive and non-intensive services depending on service

Staffed Living is one supported living option that Community Living BC (CLBC) funds to help 
eligible adults live, with support, in the community. In Staffed Living services, people who are 
eligible for CLBC and approved for this service live in a home that is typically owned or 
operated by an organization and receive support from staff who work on a schedule. People 
supported in Staffed Living often require significant support to participate in their community. 
Each home may support between 1 to 4 people and must adhere to rigorous standards under 
CARF Accreditation and Licensing (where applicable).

The successful applicant will have a minimum of a Bachelor’s Degree in Human Services or 
equivalent, and at least 5 years’ experience in mid to senior level management role(s). The

Working with Psychogeriatric Case Manager, the PSW will enhance the quality of life for individuals with responsive behaviours in hospital. The PSW is part of a team that provides behavioural support services to individual persons served, transitional management between health care providers and ongoing training for persons served, family, caregivers and clinicians.&nbsp; What You Will Do<ul><li>Act as the Primary PSW for assigned person(s) served.&nbsp; Follow the weekly schedule of required visits;</li><li>Perform and record complete and accurate observation of the person served and their responsive behaviour during initial 2 week period after being enrolled in the program. Assist in identifying triggers for the responsive behaviour;</li><li>Documents activity according to agency standards in client information system and reports observational data using specified tools (BSO-DOS, ABO’s, Standard Behaviour Questionnaire etc.);</li><li>Assists the Psychogeriatric Case Manager in the development of a Behavioural Support Plan towards the management of responsive behaviours;</li><li>Offers suggestions to the development of individual Behavioural Support Plan with special emphasis on strategies to manage responsive behaviours;</li><li>Actively practice the suggested interventions and continually adapt the interventions and strategies to ensure the best possible outcome for mitigating the responsive behaviours;</li><li>Uses the agreed upon strategies to address responsive behaviours and reports to other team members when responsive behaviours are escalating;</li><li>Accepts direction from the Psychogeriatric Case Manager regarding the Behavioural Support Plan and scheduling of visits to the person served;</li><li>Model the effective interventions/strategies to hospital staff, caregivers and/or family members using peer-to- peer mentoring;</li><li>Attend Behavioural Support Plan meetings/Discharge meetings and/or rounds etc. within the respective hospitals;</li><li>Participates in multi-disciplinary conferences, care conferences for the person served, and staff development conferences;</li><li>Shares responsibilities for care of the person served by working together with other staff members;</li><li>Actively assist and support the transition of persons served to their next care destination;</li><li>Meets the needs of&nbsp;
