The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The Personal Support Worker will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.&nbsp; What You Will Do<ul><li>Support of the physical and mental health requirements of client</li><li>Establish a working rapport with clients that supports their dignity, encourages personal empowerment, and facilitates addressing their psychosocial, behaviour management and medical needs.</li><li>Support clients with reminders related to taking their medications at the scheduled time(s) and in accordance with their physicians' instructions.</li><li>Adhere to residence/program established medication system procedures, including ordering, recording and reminding clients about the administration of their medication as well as the self- medication program.</li><li>Monitor and support clients with medical problems and illnesses, including cleaning areas which require immediate attention, i.e. linens, floor coverings, etc. for the comfort and support of the clients. Further, record illnesses and circumstances related to illness, and arrange for medical/emergency intervention in liaison with the Senior PSW and Program Manager.</li><li>Encourage and assist clients in the following activities of daily living:<ul><li>laundering and ironing of personal clothing</li><li>dressing/undressing, and ensure clients are dressed appropriately,</li><li>personal hygiene including grooming, shaving, oral hygiene, nails and bathing,</li><li>ambulation, toileting and transferring support,</li><li>at mealtimes, meal preparation, serving, feeding, cutting food, etc.,</li><li>care for incontinent clients, make client's bed,</li><li>shopping.</li></ul></li><li>As a means of promoting a normalized lifestyle, support clients with daily living chores such as bed making, dusting, sweeping and generally keeping their living space in a clean and orderly fashion.</li><li>Provide support with managing behaviours and problems arising from cognitive issues.</li><li>Provide social support in areas such as conversation, making phone calls, writing letters, facilitating interest in hobbies and other recreational activities.</li><li>Escort to medical appointments, visiting friends and other identified needs for social interaction in the community as needed.</li><li>Maintain current and accurate records of the provision of care services and document behaviours for individual clients in accordance with established procedures.</li><li>Participate in regularly scheduled meetings with clients and/or other members of the staff team for the purpose of developing goals of support for individual clients. In addition, record all incidents using the appropriate forms. Client Services Manager – Adult Day Program (ADP) Full time | Hybrid | Aurora I Travel required across York Region &amp; South Simcoe&nbsp;Position Overview:Reporting to the Vice President, Client Care &amp; Services, you will effectively manage the day-to-day operations of CHATS’ Adult Day Programs and ADP@Home Spoke, including supervision of staff, students, and volunteers. You will be responsible for ensuring high-quality, person-centered care through intake, assessments, program planning, case management, and transitions within CHATS’ Wellness Framework. This position is accountable for program standards, service targets, and quality outcomes across all CHATS ADP sites across York region and South Simcoe.Full-time position, 35 hours per week, 8:30am - 4:30pm - Monday to Friday with On-Call expectations and occasional work outside of business hours including evenings/weekends to attend organization-related events and flexibility for support during emergency situations.Hybrid work environment with base location as CHATS Head Office in Aurora.Key Responsibilities of the role:<ul><li>Lead the delivery of a high quality and safe adult day program within a multi-disciplinary team and an integrated Wellness Framework model. &nbsp;</li><li>Provide innovative leadership to maximize program efficiency, staff satisfaction, and quality care in the delivery of the Adult Day Programs.</li><li>Provide supervision overseeing individual and team performance.</li><li>Develop and nurture self-managed teams to achieve seamless integrated delivery of client service</li><li>General Program and organizational responsibilities.</li></ul>What you bring to the role:<ul><li>Degree or diploma in Management, Health or Social Services, or equivalent, combined with 5 years’ experience in community support services.</li><li>Demonstrated program development and management experience.</li><li>Proven experience building partnership and fostering community development within diverse communities.</li><li>Experience in managing, supporting and developing self-managed teams.</li><li>Exceptional communication, interpersonal and leadership skills to build high performance teams.</li><li>Excellent skills in risk, crisis and conflict management including customer relations/service.</li><li>Financial management skills, budget and fiscal monitoring and reporting.</li><li>Program coordination and case management experience required with an understanding of the principles of recreation/therapeutic programming</li><li>Participate in CHATS quality and safety initiatives and/or committees as required.&nbsp;</li><li>Annually participate in the development of the strategic direction of CHATS, in collaboration with the management team.&nbsp;</li><li>Demonstrated success in developing, implementing, and evaluating services and programs.</li><li>Excellent computer skills with proficiency in MS Word, Excel, Outlook and database applications (GOLDCARE).</li><li>Ability to work outside regular business hours (8:30AM-4:30PM) when required&nbsp;</li><li>A vehicle and valid Ontario Driver’s License required.
