students and families in addressing the social, behavioural, and emotional factors which impact a

This role follows a 10âmonth work schedule aligned with the academic year, working from 
late August to late June each year and is off during the summer. 
 
Given the dynamic and collaborative nature of our school environment, this role is an on-campus 
position requiring to be on-campus five days a week. Candidates should be prepared to

KEY RESPONSIBILITIES 

â Consultation with students, parents and faculty; provide guidance and assistance in 
support of the psychosocial health and well-being of students

â Support the enhancement of partnerships with community agencies and outside 

professionals to ensure optimal support for students and the school community; liaise

QUALIFICATIONS AND SKILLS 

â Masters degree in Social Work (or equivalent) and membership in good standing with the

About the RoleThe Waterfront Regeneration Trust is small, hard-working, highly collaborative charity that leads work to complete, expand, enhance and promote the Great Lakes Waterfront Trail. The Trail is a foundational part of the Trust’s mandate to regenerate waterfronts. Celebrating its 30th anniversary in 2025, it connects 177 communities over 3600 km. Our key promotional and engagement program is an annual multi-day cycling event called the Great Waterfront Trail Adventure (GWTA). The highly-rated event showcases a different section of the Trail every year in collaboration with our community partners. Job Posting – Internal/External&nbsp;POSITION:&nbsp;After Care Worker&nbsp;LOCATION: OutreachHOURS:&nbsp;8am – 4pm Monday - FridayNa-Me-Res&nbsp;(Native Men’s Residence), located in Toronto, is a diverse, multi-service organization.&nbsp; Our Mission Statement is: Through a culture-based approach that addresses the holistic needs of its clients, Na-Me-Res’ mission is to provide temporary, transitional and permanent housing to Indigenous men experiencing homelessness in Toronto, while providing outreach and support services to the broader Indigenous homeless population.MAIN FUNCTION:&nbsp;The After-care worker will&nbsp;provide newly housed people who have who have experienced difficulty maintaining housing with the resources required to enhance the quality of their life, hence optimizing the likelihood of their remaining successfully housed.DUTIES AND RESPONSIBILITIES:<ul><li>Conduct client assessments and develop case management plan</li><li>Receive referrals, review case summaries, and create client files</li><li>Participate in joint meeting with client and Housing Worker</li><li>Develop short- and long-term goals to maximize a client’s chances of remaining successfully housed</li><li>Assess client’s health and identify furnishing needs</li><li>Meet with client on a regular basis to monitor progress toward goals</li><li>Assess and revise case plans as required</li><li>Provide tenant/housing provider mediation service as required and any other interventions which may be necessary</li><li>Rehouse clients if needed</li><li>Participate in Streets to Homes monthly case conferences </li><li>Maintain up-to-date file on each client including case notes and referrals</li><li>Provide all reports to City of Toronto and Na-Me-Res as required</li><li>Other duties as identified by the Manager of Outreach Services</li></ul>EDUCATION &amp; EXPERIENCE:<ul><li>A degree or diploma in&nbsp;social services or other related discipline or equivalent</li><li>Several years of related work experience</li><li>A valid Ontario Driver’s License and clean driving abstract is an asset</li><li>Provide police check as a condition of employment</li><li>Knowledge and ability to work with computers</li><li>Ability to write and communicate clearly and effectively</li><li>Ability to prioritize and manage multiple jobs effectively</li><li>Successful experience working with a diverse population</li><li>Knowledge of Native traditions and culture is an asset</li></ul>Na-Me-Res encourages Indigenous applications (First Nation, Metis and Inuit) to apply.
