We are seeking a strategic and relationship-oriented Coordinator to lead our partnership 
engagement and community outreach efforts. This role plays a key part in shaping and 
managing collaborative relationships with community agencies, and sector partners that

POSITION OVERVIEW The Health Equity Promoter is a core part of PPT’s Black Health Team which includes the Black Reproducï¿½ve Health Iniï¿½aï¿½ve and the Black 2SLGBTQ+ Mental Health Iniï¿½aï¿½ve. This role works closely with the Manager of Health Equity, reporï¿½ng under the Director of Community Programs, and will assess community health needs, develop, implement and evaluate health promoï¿½on programming provided by Planned Parenthood Toronto. They will work from our main Toronto office and be responsible for developing programming and facilitaï¿½ng workshops offsite across the GTA, with a specific focus on our mobile or satellite spaces in North York and Scarborough. Informed by social jusï¿½ce, community development, health equity and the social determinants of health, this posiï¿½on works to empower Black youth through engaging, trauma-informed educaï¿½on and peer- based mental and reproducï¿½ve health programs. This posiï¿½on requires a meaningful understanding of Reproducï¿½ve Jusï¿½ce and the specific impacts facing Black, racialized and 2SLGBTQIA+ youth. This role also involves working with a Black Youth Advisory Commitee to create capacity building resources for service providers working with Black and QTBIPOC youth. The successful candidate will have demonstrated experience championing culturally safe approaches to health equity through data-informed program design, advocacy, and systems change. Central to this posiï¿½on is building relaï¿½onships with community stakeholders and providing educaï¿½on and resources that address social determinants of health, with a focus on heath inequiï¿½es impacï¿½ng Black youth. Support advocacy related to community health needs, including parï¿½cipaï¿½ng in community forums and networks • Lead in the creaï¿½on and implementaï¿½on of health equity programs and iniï¿½aï¿½ves. Monitor and evaluate program effecï¿½veness and adjust strategies as needed. • Work closely with healthcare providers, community organizaï¿½ons, and public health agencies to coordinate efforts and ensure a unified approach to health equity. • Work with the Community Programs team to develop staff and volunteer orientaï¿½on curriculum and materials and the Health Service Team to create pathways to primary care, help determine prioriï¿½es and idenï¿½fy health equity needs of Black and racialized youth. • Collect, analyze, and report data related to health dispariï¿½es and program outcomes.
