The Garry Hurvitz Centre for Community Mental Health at SickKids (GH-CCMH) is a child and youth mental health center integrated with The Hospital for Sick Children (SickKids). GH-CCMH is also a member of the Thriving Minds Mental Health Access Collaborative with CAMH and SickKids and a core service provider in the Toronto sector of infant, child and youth mental health services funded by the Ministry of Health (MOH). GH-CCMH provides prevention, early intervention and health promotion services across the city, funded by MOH and other sources. As such, the Centre has an ambitious agenda to provide the highest quality, evidence-based care, offering a range of services from prevention and early intervention to counseling and therapy, assessment and intensive supports. Services are offered in person and in virtual format. We are currently seeking applicants for a Community Parent, Family Home Visitor to provide community-based primary prevention and early intervention services to children (0-6) and families in the St Jamestown Community through the Garry Hurvitz Centre for Community Mental Health at SickKids. This role focuses on community engagement, peer leadership and co-designed programming serving as a bridge between families, community members, and clinical services to improve access, responsiveness and culturally relevant support.

Key Responsibilities:

Community and Home-Based Services

  • Provides community-based primary prevention and early intervention services to children (0–6) and families in home and community settings
  • Builds parenting capacity by strengthening skills, reinforcing family strengths, and connecting families to resources
  • Applies culturally responsive, inclusive, and trauma-informed practices in all interactions
  • Collaborates with community members and agencies to co-create programming for diverse populations, including newcomer families
  • Promotes safe, supportive environments that enhance child well-being and reduce risk

Community Engagement and Peer Leadership

  • Actively engages families and residents of the St. Jamestown community to build trust, identify emerging needs, and strengthen community connections
  • Serves as a peer leader and community ambassador, bringing lived experience and community knowledge into program design and delivery
  • Co-develops and facilitates that respond to identified community needs
  • Elevates community voices to inform program planning, service delivery, and organizational initiatives
  • Supports the development of community leadership capacity through mentorship, participation, and peer support
  • Acts as a key informant, sharing insights on community trends, emerging issues, and engagement strategies

Other

  • Complies with the organization’s vision, policies and procedures
  • Performs other tasks as assigned by management
  • Maintains accurate and timely documentation of services, engagement, and program participation
  • Prepares materials for programming and supports basic outcome tracking

Qualifications:

  • High School graduation and a minimum of one years’ related experience or equivalent combination of education and experience
  • Good verbal and written communication skills to effectively deal with sensitive and confidential child and family issues
  • Good problem-solving skills to identify and report serious issues
  • Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work
  • First Aid, CPR and Behaviour Management Techniques mandatory
  • Knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as office (telephone, scanner, photocopier and fax) and kitchen equipment
  • Ability to work flexible hours, including evenings, weekends and on-call duties
  • Ability to travel to conduct home visits and community program sites.
