Fundraising Event Ambassador – Volunteer Role Description 

About DCHC 
 
Durham Community Health Centre (DCHC) is a registered, charitable organization that provides 
integrated, accessible, and equitable community based primary care, wellness services, and health 
education to Durham community members who face multiple barriers to their health and well-being. 
DCHC also provides equity-based programs and services that focus on priority populations such as 
Indigenous, Black, the 2SLGBTQI Community, Newcomers to Canada, Seniors, and Unattached patients 
(i.e., those without a family doctor), to name a few. DCHC ensures Durham community members receive 
not only sick care, but preventive care as well. 
 
Our care model is built around interprofessional teamwork and equity-based approaches, ensuring every 
client receives coordinated, person-centered care across clinical, wellness, and education services. 

Key Responsibilities 
 
DCHC is seeking enthusiastic volunteers to help promote our fundraising events. Currently, we are 
looking to promote our Annual Golf Classic: Driving Community Health Foreward - our signature 
fundraising event that supports the vital work we do in the community. 
 
This year, our Golf Classic will take place on Tuesday, September 16th, 2025, at Deer Creek Golf Course in

As a Fundraising Event Ambassador, you will play an important role in spreading awareness about 
fundraising events and the impact of DCHC’s work. Volunteers will play a key role through: 

 • Distributing pre-written letters to local businesses to request prizing for events (e.g., gift cards, 
prize baskets, etc.) 

 • Visiting and/or calling local stores to inquire about displaying promotional flyers in storefronts, 
etc. 

 • Sharing event details and DCHC’s mission with the community 
 
What to Expect: 

 • Volunteers will receive an onboarding and orientation to prepare for the Ambassador role 

 • Volunteer must be comfortable completing tasks independently 

 • Ability to travel within the local area is required 

 • Volunteer hours can begin as soon as onboarding is complete and can continue through late 

August to early September 

 • Flexible scheduling to fit your availability 

 • Volunteers will report into the Manager, Philanthropy and Partnerships and will be further 

supported by the Lead, Community Engagement

Job Purpose: 
 

As a dynamic and welcoming member of our team, the Case Management Worker will provide intake 
interviews and follow-up support services with new members. This position provides front line case 
management and counseling to new members, refers them to appropriate programs and resources and 
provides ongoing follow up support, in addition to providing group facilitation for wellness programs 
and emotional support groups. 

 
Reports to: 
Executive Director 

 
Primary Responsibilities: 
Intake Procedure/Support Services 
 • Provide Intake interviews for new adult members. 
 • Coordinates individual follow up appointments, member outreach and support for drop-in visitors. 
 • Ensures that membership and intake forms are completed, as well as all relevant referrals to peer 

support and program registrations. 
 • Manage an individual caseload providing individual emotional support to families and individuals as 

required. 
 • Provide support and resources for Caregivers. 
 • Facilitate group programming or seminars as requested by the Program Coordinator and Executive 

Director. 
 • Collects statistics for services provided including providing a monthly report to the Board of 

Directors. 
 

Follow-Up Procedure 
 • Follow procedures to track follow-up phone calls, support needed after a group finishes etc. to 

ensure accountability, continued support and member satisfaction. 
 • Ensure client notes and database entries are conducted in a timely manner and written in clear, 

concise, and non-judgmental language. 
 
 

Resources 
 • Compile, create and expand resources for members as requested by the Program Coordinator or 

Executive Director. 
 • Assist with needs assessment and development of peer support services, seminars and support 

groups as required. 
 
 

Qualifications: 
Experience 
 • 5-10 years of social work or human services counseling.

 • High school diploma. 
 • Post-secondary education or equivalent experience in Social Work, Community Support Work, 

Mental Health counselling or equivalent. 
 

Knowledge, Skills and Abilities 
 • Knowledge of MS Office applications and client database management (knowledge of Sumac/Societ 

database is an asset). 
 • Counselling skills and knowledge of community resources. 
 • Group Facilitation skills. 
 • Positive, calm, and collaborative approach, with desire to build productive relationships and an 

effective team environment. 
 • Excellent oral and written communications skills.
