In partnership with the Niagara Region Indigenous Community, Bethlehem Housing and Support Services (BHSS) will operate a Supportive Transitional Housing program for women and children in the city of Fort Erie.&nbsp; We are seeking two Community Service Navigators (CSN) who will&nbsp;provide advocacy and support to clients in the Transitional Housing Program to achieve community integration while working on a long-term plan to achieve housing stability in the community.&nbsp; &nbsp;The Navigator will work with Indigenous women and children, and those facing issues such as poverty, homelessness, domestic violence, mental health issues, addiction issues and/or family breakdown.&nbsp;In collaboration with the staff team, the program provides safe transitional housing, basic structure in independent living, support with goal setting, and personal capacity building programs.&nbsp; By creating and offering this supportive environment, clients and their families can overcome trauma, address the issues that have led to or kept them homeless, and begin to rebuild their lives through a support network.&nbsp;QUALIFICATIONS &amp; COMPETENCIES<ul><li>Diploma or Degree in Social Services or another public service-related field.</li><li>Minimum of two years in related social service experience, preferably with direct program delivery, frontline or client facing role in the areas of supportive housing, violence against women, mental health, homelessness, and/or addictions. Services are designed to be short-term, focused on comprehensive assessment, stabilization and linking to long term resources. M-DOT provides services to individuals often identified as ‘complex’, who have not responded positively to other programs or attempts to engage. The aim of M-DOT is to provide a low barrier transitional service, which meets individuals where they are at and assists in stabilizing their health, mental health and substance use.&nbsp; M-DOT works from a housing first and harm reduction approach. The incumbent for this position will have a Bachelor of Social Work Degree or equivalent from an accredited university and demonstrated experience in delivering needs-based, culturally grounded and trauma informed supports for justice involved individuals. A Master of Social Work is preferred. The ideal candidate will have a minimum of 2 years experience working with justice involved individuals. They will have lived or learned experiences interacting with vulnerable individuals, a strong knowledge of the criminal justice system, familiarity with community-based and criminal justice’s services in the GTA and specialized knowledge of the challenges that overrepresented and marginalized populations, including Black, Indigenous and racialized individuals experience in the justice system.&nbsp;&nbsp; The incumbent will have excellent conflict resolution skills, case management and facilitation skills. Expertise in Cognitive Behavioural Interventions, Motivational Interviewing and Stages of Change and identification of criminogenic factors as they relate to developing individual intervention plans is essential. The focus of work for this position will include responsibility for the development of individual participant goals and plans geared to personal development and opportunities for positive change for program participants. You will also be expected to coach/mentor and provide participants with relevant and evidence-based service interventions to promote assets, competency, and resiliency. You will assume service coordination functions while acting as an advocate and providing information and referrals to program participants. Mentoring and counselling program participants to encourage social inclusion and engaging these program participants to identify issues, needs, goals, aspirations, skills, abilities, and services needed, as well as barriers to accessing these services, is critical. As a member of the UHHS department, this position will provide support in the Corner drop-in as well, working closely with the integrated team. MAJOR DUTIES: • Conduct intake and assessment interviews with participants. • Complete detailed assessment, coordination of supports, referrals and advocacy on health issues, substance use, mental health, financial, employment, housing, legal and social supports. • Work with participants to identify and achieve self-directed goals • Maintain ongoing support in the community, including home visits, office visits, accompaniments to appointments, and meetings in institutional settings. • Monitor property issues (rent payment, health and safety, housing standards) and liaise with landlords as required • Work collaboratively and develop strong partnerships with external service providers, such as withdrawal management services, hospitals, law enforcement agencies, and housing providers • Connect clients with supports in the community based on their needs which will help them achieve and maintain stability, including support groups, life skills groups and social events. • Organize group programming that promotes well-being and recovery • Support client engagement initiatives within the agency including the volunteer program and peer development initiative • Intervene and deescalate crisis situation • Engage in the active team case management and clinical consultation meetings • Maintain accurate case management records and complete reporting required by funders and supervisor, adhering to PHIPA and TNG-SSCH policies. REQUIREMENTS &amp; QUALIFICATIONS: • BA/B.S.W plus 2 years of experience in human services, preferably as a case manager or a combined equivalent of education, lived experience and/or significant related training. • Minimum 2 years’ direct client experience working with individuals living with mental health, substance use issues, histories of trauma and homelessness • Experience, alignment and commitment to harm reduction principles and practice • Commitment to anti-racism and anti-oppressive practices • Formal training in substance use and mental health is an asset • Strong familiarity and commitment to trauma informed care • Physically able to travel consistently throughout the city on transit • Recent demonstrated knowledge and skills in assessment, crisis intervention and prevention, conflict resolution and negotiation skills • Proficient with MS Office, and web-based data entry for case management and time/labour. • Familiarity with Catalyst, Pirouette and the administration of the OCAN is an asset TNG is committed to reflecting the communities we serve and to nurturing a respectful and inclusive work environment for all.
