These include untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental health and substance use challenges. The Substance Use Hub (HUB) was launched in 2021 by Inner City Health Associates (ICHA) and the Inner City Family Health Team (ICFHT) to address the ongoing substance use and overdose crisis in Toronto's homeless population. The service provides safer drug supplies, addiction medicine treatments and psychiatric care with a pathway to primary care and wraparound supports for individuals struggling with substance use who are at the highest risk for overdose and death. The project aims to support a substantial number of individuals who have the greatest risk of overdose and death due to substance use with the goal of seeing a decrease in the number of substance-related deaths and overdoses that are reported amongst the homeless population.Purpose of the Position:The Case Manager provides direct case management support to adults with a comprehensive range of complex issues, including health, financial, housing, legal, and life skills concerns. Working from a harmful, trauma-informed perspective, Case Managers provide extensive direct one-to-one support, home visits and telephone contact to assist participants in identifying and achieving self-determined goals. Position Summary: 
The Mentoring Coordinator initiates, facilitates, supports, and monitors the mentoring relationship between a 
youth (the “Little” or “Mentee”) in need of academic support and an adult volunteer (the “Big Sister” or 
“Mentor” ). Recruiting, interviewing, screening, training, and supporting volunteers, as well as ongoing record 
keeping via database management, are key components of this position. 
 
What you will be doing: 
Case Management 

 • Conducts intakes with potential Littles / Mentees and their parent(s) / guardian(s) – explains the 
program, prepares case histories, assesses child and family needs and strengths, refers families to 
additional programs and resources as appropriate / necessary 

 • Interviews Big Sister and Study Buddy Volunteer applicants; provides assessment on suitability for the 
program; conducts training and orientation sessions for approved volunteers; manages and documents 
volunteer correspondence 

 • In accordance with Big Brothers Big Sisters’ National Standards, maintains ongoing contact with the Big 
Sister Volunteer and Little / Mentee (and their family) to ensure a supported, successful, and culturally 
responsive &amp; appropriate match; 

 • Monitors each match with particular attention to Little / Mentee’s physical, mental, and emotional 
safety &amp; wellbeing 

 • Develops and implements appropriate match support strategies as necessary (increased levels of 
monitoring &amp; support, connections to additional resources, etc.) 

 
Database / Client Records Management 

 • Ensures client and volunteer files are accurate and up to date 
 • Documents interviews, check-ins, conversations, emails, etc. with mentoring matches and produces 

written summaries, assessments, and reports

Agency policies and FOIPPA regulations 
 
Program Operations 

 • Participates in the organization of, and offers suggestions on, agency activities, special events, 
scholarship applications, camps and workshops

What you bring: 
Experience &amp; Education 

 • Minimum 2-year post-secondary diploma with related work experience (required) or a bachelor’s
degree in social sciences or humanities (preferred)

 • 1 - 3 years direct program delivery experience working with children and families, ideally in a not-for-
profit social services organization

 • Demonstrated knowledge of community-based programs and provincial support systems for youth &amp;
families is preferred

JOB DESCRIPTION: 

1. To liaise with community stakeholders through engagement with local groups in
the South Etobicoke area.

2. Recruit, train, hire, orient, supervise, coach and debrief Case Managers on a daily
basis.

3. Supervise students and volunteers and staff from in your related dept.
4. To ensure that all deliverables are achieved as required by our funders as per our

program Work Plans.
5. Assist with the completion of funding reports/applications, and research

initiatives as directed.
6. To maintain and submit monthly program statistics.
7. Liaise with funders and government representatives when required.
8. Complete annual staff performances including student placement evaluations;
9. Weekly and monthly tabulation and reporting of statistics internally and

externally (to funders as required) including housed and discharged client
documentation, monthly TGRIP data entry and six month reports;

10. Design and implementation of on-going follow-up services for clients after they
are housed;

11. Advocacy with landlords and other housing providers;
12. Liaison with other housing and supports agencies;


13. Development and Submission of reports, proposals and other government 
requirements as requested; 

14. Distribution of TTC fare and submissions related documentation to clients; 
15. Complete annual staff performances including student placement evaluations; 
16. Development of innovative strategies to assist ‘hard to house’ clients; 
17. Oversee case management and provide supportive counseling; 
18. Ability to resolve and de-escalate crisis situations; 
19. Conflict mediation relevant to housing maintenance

6. Knowledge of community reintegration barriers for formerly incarcerated 

persons, provincial court/correctional processes, housing and homelessness 
in a local context 

7. Demonstrated experience working with a diverse ethno-racial and multi-
lingual client group. 

8. Commitment to the principles of anti-racism, anti-sexism, and to challenge 
other forms of oppression. 

9. Strong verbal and written communication skills, administrative skills, and a 
willingness to work as part of a team.
