Rebound Child & Youth Services Northumberland (Rebound) Rebound is a charitable, not-for-

profit organization committed to providing high-quality, community-based programs and services

to children, youth, and families in Northumberland County. Rebound proudly leads the Youth

Wellness Hubs Northumberland in partnership with Northumberland Hills Hospital (NHH),

Community Living and Respite Northumberland, Community Health Centres of Northumberland

(CHCN), YMCA Northumberland, and the Municipality of Port Hope.

The Hub is part of the Youth Wellness Hubs Ontario (YWHO) provincial initiative focused on

providing Integrated Youth Services (IYS) to address the service gaps in the youth mental

health sectors in Ontario.

Position Summary

Rebound is seeking a passionate and experienced Care Coordinator Manager to lead the

operations of the Youth Wellness Hub Northumberland (YWHO). This is a temporary 1-year

maternity leave contract with the opportunity to make a lasting impact on youth mental health

and wellness in our community.

The Care Coordinator Manager, reporting to the Executive Director at Rebound, will lead and

supervise a multi-disciplinary team of staff in the implementation and day-to-day operations of

the YWHO Northumberland location. The Care Coordinator Manager will be responsible for

coordinating high level, youth centered, wraparound services and activities for the Hub.

As a key member of the Rebound leadership team, the Care Coordinator Manager will embody

values of care, compassion and inclusivity and approach the work with a trauma-informed,

client-centered, and solution-focused mindset within an anti-oppressive practice framework.

Roles and Responsibilities

● Provide strong leadership to an interdisciplinary team in the development, monitoring

and implementation of the integrated YWHO governance model and Core Components

to promote barrier-free access for diverse youth.

● Effectively handle the day-to-day operations of the Hub, including setting, managing and

evaluating program quality and performance indicators and lead continuous

improvement projects.

● Collaborate with the Executive Director to plan and manage program budget, identify

fundraising and sponsorship opportunities, and develop funding reports.

● Establish and maintain strong working relationships with service delivery partners,

advisory committees, youth and families.

● Lead service integration using protocols and Memorandums of Understanding.

● Oversee transitional case management for youth under 16 and rural youth seeking

services.

● Ensure safe operations in a busy drop-in environment and promote best practices in

mental health, substance use care, and social services.

● Engage youth in developing service pathways and programs, ensuring that youth, family,

and community members’ voices inform Hub development and governance.

● Maintain a diverse schedule of program supports and activities for youth wellness.

● Develop key partnerships for coordinated care and act as a liaison between community

partners, the Northumberland Action Team, and the YWHO Provincial Office staff to

ensure that all YWHO Core Competencies are met and maintained.

● Represent the Hub in the provincial YWHO Network, including attending YWHO Network

Operations meetings and collaborating with the YWHO Provincial Office and the

provincial Implementation and Evaluation team on data collection.

● Oversee the adoption and compliance of the YWHO data collection platform, My

Wellness Passport, the use of Measurement Based Care by employees and partner

agency staff.

● Ensure consistency among staff and service delivery partners in assigning client

identifiers and overseeing data collection/data input processes, ensuring compliance and

privacy within a Personal Health Information Protection Act (PHIPA) framework.

● Providing reports to the Collaborative Partnership Team (CPT) regarding Hub operations

and highlighting successes and potential risks.

Working Conditions ● Flexibility in work hours, including weekends/evenings for program delivery and events.

● Home office or private space required for maintaining client confidentiality if remote work

is necessary.

● Travel to program locations and events.

Qualifications, Experience, and Requirements

● Master’s degree (preferred) in Social Work, Child/Youth services, or related health field.

● Experience in mental health, substance use, or clinical management, preferably in

youth-focused non-profit community agencies.

● Understanding of local community issues impacting diverse and equity-deserving youth

and available community resources.

● Program development and project management experience is an asset.

● Ability to manage staff, provide mentorship, and ensure accountability in a client-focused

environment.

● Knowledge of health equity approaches, including anti-oppressive practice, 2SLGBTQ+

inclusion, trauma-informed care, and harm reduction.

● Strong organizational, interpersonal, and communication skills.

● Ability to foster a collaborative, team-oriented environment.

● Valid driver’s license and reliable vehicle with minimum $2,000,000 insurance liability.

● Satisfactory Criminal Record Check, including Vulnerable Sector search.
