Support Worker (Full Time)

Summary:

The Support Worker will be working in the Supportive Independent Living Program (SILP) under the direction of the Manager, Program Development and Delivery. The Support Worker will be supporting clients by responding to their inquiries and concerns, liaising with housing providers, providing crisis intervention when necessary, and overseeing case management for clients residing in the Supportive Independent Living Program.

Tasks and Responsibilities:

  • Provide psycho-social and practical support to clients residing in Bruce House designated apartments with local social housing providers (OCH, CCOC) with the goal of helping them maintain housing
  • Manages the waiting list for clients waiting for Rent Geared to Income housing with Bruce House.
  • Responsible for Case Management of clients in the SILP program alongside the Manager, Program Development and Delivery as well as the Program Team.
  • Plans regular check-ins with clients over the phone, in person or by email.
  • Works together with the Programs Team to assist with social programs available to all Bruce House clients, based on their needs and interests.
  • Provide statistics and documentation for activities of the SILP; participate in program development activities with the Programs and Services Team.
  • Manages the client rents and utilities tracking for the SILP Program.
  • Supports the Executive Director with the SILP Rent Log finances.
  • Attends Social Housing Networking meetings, including advocacy for homelessness issues when required.
  • Support other Bruce House programs and services, as required.
  • Other duties as required.

Education and Experience:

  • A degree or diploma in health or social services or significant relevant experience.
  • Knowledge and experience of psycho-social issues related to HIV.
  • Knowledge of housing, community and social service resources.
  • Knowledge of Microsoft Office programs and client service reporting tools.
  • Knowledge of social determinants of health, and their impact on marginalized communities.
  • Knowledge of harm reduction is an asset.
  • Experience working with diverse communities impacted by HIV.

Language and other:

  • Bilingual (English and French)
  • Valid driver’s license and access to an automobile.
  • Occasional evening meetings and weekend community events.

  • Full time permanent position Monday to Friday.
