Position Overview:

Reporting to the Manager, Health Services, the Supervisor is responsible for coordination

and oversight of the interprofessional healthcare providers to ensure high-quality, client-

centred, integrated, and responsive health services programs services in alignment with

Vibrant Community Health’s mission, vision, goals and values. The Supervisor will work

Interdependently with the Clinical Administrative Supervisor and Health Services Manager to

ensure effective client flow.

In this role, you would contribute to an integrated team-based model focused on creating

impactful care pathways and setting new standards in patient care. You will be pivotal in

fostering an inclusive healthcare setting where equity-deserving people living with physical

disabilities or other complex health issues experience the highest quality access to care. By

Summary of Duties:


• Provides supervision, direction, oversight and support for the service delivery

activities of the interprofessional healthcare team ensuring an integrated,

collaborative client-centred approach


• Sets clear goals for the teams in alignment with medical directives, organizational

priorities and funder expectations and leads by example to engage the team to

achieve goals


• In collaboration with teams, identifies gaps in service and presents recommendations

to address these gaps


• Leads the development of adequate staffing levels and schedules to ensure

maximization of service delivery


• Monitors eligibility and priority criteria to support timely access of programs and

services to clients; makes recommendations to improve access.


• Support the manager to resolve escalated staffing or client issues.


• Participates in the development, implementation, monitoring and evaluating program

and service delivery and key performance indicators to inform business and service

improvement opportunities.


• Coordinates the maintenance of clinical office equipment, and medical supplies and

inventory including the purchasing and procurement and management of external

suppliers/vendors.


• Instructs staff on safe work procedures, occupational health and safety policies and

requirements to ensure the safety of staff and clients and monitors compliance.


• Supports the development and implementation strategies and initiatives to promote a

safe work environment including training, communications and increasing

awareness.


• Review and ensure that the allied health team is adhering to clinical documentation

practices and provide feedback to address gaps.

Qualifications/Skills:


• Minimum 2 years of progressive responsibility in a leadership position at the

management level in a community-based health care or non-profit environment


• Degree in public health, health administration or a related field, or equivalent

combination of education and experience


• Experience managing interdisciplinary teams


• Experience working in a unionized environment.


• Working knowledge of clinical evidence-based practices and health promotion, illness

prevention and wellness models in a community setting.
