POSITION TITLE: Street Outreach Nurse
STATUS: part-time, fixed-term contract until July 31, 2026
# HRS/WK: 20-24 hrs/week, dependent on funding
FORMALLY REPORTS TO: Executive Director All Saints Church Community
ADDITIONAL REPORTING: Director, Common Table, Church of the Redeemer; Incumbent,
Holy Trinity Church
SALARY: $38/hour + benefits
START DATE: September 22nd or until filled
BACKGROUND:
All Saints Church, Church of the Redeemer, and Holy Trinity Church are working in partnership
to hire a part-time Registered Nurse for a fixed-term contract. The three partner organizations
provide support to some of Toronto’s most economically disenfranchised people: people who
struggle with homelessness, mental illness, addiction, and other challenges, and offer a range
of services to respond to the needs of the community.
PURPOSE:
To provide empathetic nursing care while maintaining the dignity and respect for the clients
encountered in the programs and the communities of All Saints Church-Community Centre,
Church of the Holy Trinity and Church of the Redeemer, Anglican Churches in downtown
Toronto.
RESPONSIBILITIES:
• Provide nursing care for individuals in the programs and communities of All Saints Church-
Community Centre, Church of the Holy Trinity and Church of the Redeemer.
• Provideprimary care, wound care, diabetic care, and harm reduction.
• Liaise with other health care providers in providing continuous care and follow-up.
• Provide nursing services, such as communicable disease support for those with HIV and
tuberculosis, sexually transmitted infection, in addition to follow-up with the clients.
• Provide supportive mental health care including access to drug and alcohol rehabilitation
and mental health follow-up.
• Provide well-woman care and make appropriate referrals when necessary.
• Ensure continuum of care through basic note-taking.
• Ensure cleanliness of space care is offered access to appropriate supplies is available. This
will be supported by others as appropriate.
• Facilitate access to healthcare services, such as primary care or harm reduction strategies
for clients.
• Assist clients with health-related prevention issues, including vaccine hesitancy.
• Assist elderly houseless individuals with physical limitations, or who experience cognitive
decline or dementia, to navigate hospice, palliative care, and/or long-term-care referrals.
• Attend and contribute to staff meetings as required.
• Retain and maintain accurate records onsite as required by the College of Nurses of Ontario.
• Accompany members of these communities to appointments or the hospital as time permits.
• Lead relevant education sessions with members of the programs, Church communities, and
neighbourhood communities.
REQUIRED SKILLS, EDUCATION AND QUALIFICATIONS:
General
• Ability to work with vulnerable individuals whose lives are characterized by poverty. These
individuals are often experiencing homelessness, living in homeless shelters, or are
underhoused.
• Experience working from a harm-reduction and trauma-informed approach
• Demonstrated ability to work with people from diverse experiences and life situations.
• Ability to enter a variety of different settings and to connect with people who have a wide
range of beliefs, values, and life situations.
• Willingness and ability to travel between the three sites.
• Ability to work both independently, and collaboratively as part of a team.
• Excellent communication and listening skills.
• Flexible, patient, and relational.
Professional
• A member in good standing with the College of Nurses of Ontario.
Education/Certification
• Registered Nurse: Bachelor of Nursing
• Current Licenses, Certifications and Credentials
• Understand the determinants of health and can use this as a framework to create
appropriate strategies for optimum client health.
• Have experience using a harm reduction approach while working in community health,
mental health, trauma and/or with marginalized people.
Considered an Asset:
• Experience working in settings interacting with marginalized/homeless individuals.
• Experience working in palliative care.
• Training in trauma-informed care preferred.
• Skills and experience in conflict management and de-escalation.
