Reports to: Social Worker Team Lead
Terms: Permanent Full-Time (35 hours/week or 1 FTE)

POSITION SUMMARY

TAIBU Community Health Centre provides primary health care and related services for Black populations across the Greater Toronto Area as its priority population and residents of the local community of Malvern. Recognizing that systemic oppression has fostered conditions of ill-health with Black communities, we strive to deliver these services through intersectional, equity based and culturally affirming practices which promote holistic wellness, health education, and prevention.

You will be part of a high functioning interdisciplinary team of health and social service professionals providing high quality, accessible and sustainable healthcare programs and services to the community in partnership with other community-based organizations to address individual, family and community needs. TAIBU is committed to creating a sustainable Community of Practice and becoming a Centre of Excellence in Black Health Care.

The Social Worker/Therapist works within the context of the Centre’s mission and strategic directions, to provide community, mental and clinical health promotion to Black identifying children, youth and their parents. The Social Worker/Therapist will be responsible for providing individual psychosocial assessments, referrals, counseling/therapy, case management, psycho-educational groups and advocacy for clients.

RESPONSIBILITIES

Client Care
• Provide psychosocial assessments by gaining an understanding of health risks based on the Social Determinants of Health framework (e.g., socio-economic position, housing, educational, financial and legal needs)
• Ensure comprehensive treatment for clients by providing information, counseling/therapy and when necessary, referring them to other community health professionals and/or act as a client advocate
• Provide crisis intervention and/or specialized treatment to complex cases
• Develop and deliver programs and organize groups for identified priority populations
• Ensure appropriate treatment is delivered by maintaining client records and assessment notes and correspondence within the safeguard of confidentiality
• Perform some case management functions

Community Resource
• Act as resource person to TAIBU by compiling information on community resources and facilitate access to those resources by liaising with community groups, and other health/social service agencies
• Keep abreast of major innovations and changes in social policy which affect the well being of clients
• Engage in community outreach to advocate for identified community needs

Administration
• Conduct pre- and post-evaluation of psycho-educational groups offered and write
program reports as per the Centre’s policy
• Keep general participant information, records of attendance and format of programs
and any other presentations as required by the Centre

Health Promotion
• Assist in the identification and assessment of health needs and advise the Centre
on appropriate responses
• Participate in the implementation of health education/promotion programs

Accountability
1. This position reports to the Social Work Team Lead
2. The Social Worker has accountability to the standards of practice of the profession and the College of Social Workers and Social Service Workers
3. The Social Worker is accountable to meet the requirements of any accountability agreement between Ontario Health and TAIBU CHC

Qualifications
• Master’s Degree in Social Work or Counseling/Therapy discipline from a recognized
University
• Current member of the Ontario College of Social Workers and Social Services
Workers or other regulated psychotherapy regulatory body
• Proven skills in psychosocial assessment, counseling/therapy, crisis intervention,
case management and referrals
• Two to four years’ experience of working with racialized and marginalized
populations in community-based settings
• A solid understanding of the Child-Welfare system and it’s impact on Black
individuals and families
• Through knowledge and understanding of health, social service and community
development agencies
• A commitment to an interdisciplinary team approach to coordination of health and
social services
• Proficiency in current assessment, counseling/therapy and psychosocial counseling
techniques for working with individuals, groups and communities
• Excellent communication skills, both oral and written
• Experience of working with children, youth and care givers
• Sound group facilitation skills
• Demonstrated experience in effective time management and organization skills
• Experience in group work and program development, implementation, monitoring
and evaluation
• Excellent communication skills (written and verbal) and group facilitation skills
• Ability to work independently as well as part of multi-disciplinary teams
• Person with lived experience
• Knowledge of the community; its members, businesses, and resources
• Access to a car an asset
• Proficiency in French a very strong asset

Compensation:
• $65,625 – 79,066 + Competitive benefits
• Expected salary adjustment in the near future – based on Ministry communication
• New recruits at TAIBU CHC will be enrolled in the Healthcare of Ontario Pension Plan (HOOPP)
