Social Worker

Join the Taddle Creek Family Health Team (TC FHT) as a Social Worker, where you’ll be an integral part of an interdisciplinary health care team. In this role, you will provide patient-centered care through psychosocial assessments, short-term counseling, education, advocacy, case management, and group facilitation. You’ll collaborate with primary care providers to support patients with anxiety, depression, trauma, mental health disorders, and other challenges, including homebound patients. Ensure comprehensive care delivery while adhering to TC FHT policies and professional standards.
