Position: Social Prescribing Navigator – IPCT Expansion

Reports to: Director of Primary Healthcare Services

Terms: Full-Time (35 hours/week)

POSITION SUMMARY

The Social Prescribing Navigator is responsible for enhancing service coordination and connecting clients to various social services and non-clinical programs needed to manage their health care goals. Working within the philosophy of TAIBU Community Health Centre, the Navigator will work within an inter- and multi-disciplinary team of professionals to ensure clients’ access the services and programs necessary to maintain and improve health outcomes. The role will also participate in the client intake, assessment, care plan and referral process within the clinical team. The Navigator will be called upon to establish formal referral pathways, develop a data base of local and internal community-based programs and to explore and develop partnerships. The Navigator will work within an anti-racist/anti-oppressive framework and will work from a determinant of health and client-centred lens.

SPECIFIC RESPONSIBILITIES

  • As a priority attach community members to primary healthcare services at TAIBU and within the Scarborough Collaborative IPCT expansion team.
  • Work closely with Locally Driven Population Health Model (LPDHM) ambassadors in advancing health and wellbeing for priority population and clients served.
  • Establish formal pathways for connecting primary care clients within the project’s priority population with social and non-clinical services.
  • Act as liaison, resource and navigator for the Scarborough Collaborative clients by helping to assess and connect clients to programs and social supports based on social prescription referrals, client interests and goals and any existing barriers clients may face.
  • Facilitates non-clinical Social Prescribing referrals and assist in managing patient referrals to the appropriate health specialties and community services where indicated
  • Maintain appropriate and timely communication with members of the primary care (including allied health) community and systems change and administrative teams
  • Maintain a repository of internal and external community-based resources, external agencies and partners
  • Assists in the development of appropriate quality assurance and improvement mechanisms for the social prescribing program
  • Makes sound decisions based on knowledge and full analysis of the presenting situation or problems
  • Shows initiative and suggests and/or participate in quality improvement initiatives that may benefit or improve the service.
  • Initiates research and prepares reports regarding client care and healthcare access, identifying needs within the community and participating in the development of new programs which address those needs
  • Participates on committees, advocacy efforts, case conferences and planning meetings as needed
  • Other duties as assigned or needed to be successful in achieving the objectives of the role

Administrative Duties

  • Share administrative and clinical maintenance tasks with other centre staff. These may include compiling statistical data, updating records, peer review of client records.
  • Maintains appropriate patient records and assures all encounters are entered into the Centre’s EMR
  • Participate/lead in/ research initiatives

Experience & Education

  • University Degree in Health or Social Sciences field with a minimum of three years work in community health or social sciences settings
  • Experience working within local health care system and extensive knowledge of community resources relevant to the priority population (i.e. the Black community)
  • An understanding social determinant of health and systemic barriers to health equity
  • An understanding, patient individual who is sensitive to people, with a variety of health, social, cultural and economic needs
  • Enjoys working in a multidisciplinary team environment
  • Excellent interpersonal, oral and written communication skills
  • Experience in the use of computers and appropriate software applications
  • Enthusiasm and interest in working with TAIBU priority populations
  • An interest in developing new skills relative to identified needs of clients

Accountability

  1. This position reports to the Director Primary Care Services
  2. The Social Prescribing Navigator is accountable to meet the requirements of any accountability agreement between Ontario Health and TAIBU
  3. Adhere to all policies and procedures of TAIBU CHC

  4. Excellent communication skills (written and verbal) and group facilitation skills are an asset
  5. Proven consultation and collaborative skills
  6. Ability to work evenings and occasional weekends
  7. Ability to work independently as well as part of multi-disciplinary team
  8. Proficiency in English is a strong asset.

Compensation:

  • $64,772 – 78,038 + Competitive benefits
  • Expected salary adjustment in the near future – based on Ministry communication
  • New recruits at TAIBU CHC will be enrolled in the Healthcare of Ontario Pension Plan (HOOPP)
