Background: Flemingdon Health Centre (FHC) is a not-for-profit Community Health Centre serving Flemingdon Park, Fairview and Thorncliffe Park. Guided by a vision of Strong Healthy Communities, FHC provides health and community services grounded in health equity, inclusion, community engagement, accountability, excellence and collaboration.
Position Summary:
FHC is hiring a Registered Nurse (RN) to be deployed and work at Unify. This is part of an exciting initiative where FHC is acting as a lead organization to support the delivery of primary care and integrated social services.
The Registered Nurse will be an integral member of the interdisciplinary team, providing comprehensive, client-centred nursing services. Working within their full scope of practice and in accordance with the standards and requirements of the College of Nurses of Ontario (CNO), the Registered Nurse will provide nursing assessment, health assessment and monitoring, care planning, health education, clinical support, care coordination, health promotion, referrals, and navigation of community and health resources. The Registered Nurse will help reduce barriers to care and address the social determinants of health while supporting clients in achieving improved health and well-being.
Deployment Role – Employment Model: While the Registered Nurse will be hired and remain employed by FHC, the incumbent will work as part of the Unify care team and follow Unify’s day-to-day site operations, schedules, and clinical workflows. Day-to-day direction will be provided on site at Unify, while formal reporting and accountability will remain with FHC.
Responsibilities Include:
Direct Client Care:
Provide comprehensive primary care within RN scope (expanded role).
Coordinate care across the Health Centre and external providers.
Provide telephone triage, advice, and follow-up on results and prevention needs.
Deliver client education and health promotion during assessments and groups.
Provide home visits and support for home-bound clients, as needed.
Health Promotion and Education:
Work with internal teams to deliver targeted health education and support community development.
Represent the Health Centre in community projects and partner activities, as needed.
Support outreach program development based on community needs.
Contribute to policies and strategies that advance public health and address determinants of health.
Apply evidence-based practice aligned with evolving community needs.
Organizational Responsibilities:
Participate in quality improvement, audits, accreditation, committees, and team meetings.
Support annual planning and evaluation of program goals.
Identify priority populations and help implement effective services.
Maintain knowledge of community services and make appropriate referrals.
Teach/supervise learners and support volunteer placements, as needed.
Participate in professional development and staff in-service training.
Perform other duties as assigned.
Skills and Qualifications:
RN in good standing with the College of Nurses of Ontario (CNO)
Bachelor of Science in Nursing (BScN)
>1 year in primary care, including:
o Assessment and triage
o Chronic disease management (e.g., diabetes, hypertension, asthma)
o Medication/treatment administration (per medical directives)
o Symptom monitoring and clinician updates
o Women’s health screening and contraception counseling
o Travel health advice and pre-travel immunizations
o Pediatric and/or perinatal experience (asset)
Strong assessment, health promotion/education, counseling, and community development skills
Ability to work effectively on an interdisciplinary team
Ability to work flexible hours, including evenings and weekends.
Additional languages, preferably Spanish or Arabic, and EMR/Practice Solutions experience are assets.
Working Conditions & Physical Requirements:
Incumbent will be expected to work some evening and/or weekend hours based on project demands.
