POSITION SUMMARY

TAIBU’s Diabetes Education Program consists of a specialized inter-professional team with a focus on providing Diabetes Education and services for residents of Malvern and the Black Community across the GTA.

The Registered Diabetes Nurse Educator functions interdependently with other members of the Diabetes Education Program, clinical and community program teams within a health promotion framework. The Nurse Educator provides education to clients through individual and group counseling to assist in the transference of knowledge to effectively self manage their diabetes. The Nurse Educator will possess a sound knowledge base around the prevention, care and management of diabetes. She/he will use a variety of teaching methods and principles of education to assist clients to gain necessary knowledge, skills and attitude for effective self management of their diabetes/pre-diabetes.

All regulatory and statutory requirements of the profession will be faithfully followed by the Nurse Educator. Internal accountability procedures will also be completed in a timely manner as required. The Nurse Educator will work within an anti-racist/anti-oppression framework.

RESPONSIBILITIES

Client Interaction

  1. Provide a thorough assessment of clients within the nursing scope of practice taking into account their level of knowledge, readiness for change, socio-economic status and cultural background
  2. Develop and support client centered health care plans
  3. Plan, implement, facilitate and evaluate innovative diabetes group education sessions in the community
  4. Recommend appropriate referral to primary care providers and allied health professionals within the community and provide follow up with individuals as necessary
  5. Regularly monitor clients within scope of practice as required in order to effectively evaluate objectives and outcomes

Outreach and Advocacy

  1. Outreach in collaboration with other TAIBU staff to identify target population and to look and book community locations as host sites for education programs
  2. Plan, organize, coordinate and engage actively in health promotion activities by providing outreach screening and presentations in targeted population areas on and off sites in conjunction with other service providers especially towards diabetes prevention and management
  3. Participate and facilitate advocacy initiatives to support individuals and identify gaps in care and/or services
  4. Identify the needs of individual clients and communities and tailor TAIBU’s services to best suit their needs

Partnership Development

  1. Identify and develop positive relationship with community agencies, resources and stakeholders to better support client needs and refer them to appropriate services
  2. Actively participate in the promotion of TAIBU’s inter-agency referrals
  3. Establish and maintain relationships with diagnostic company representatives with regard to current diabetes supplies and teaching materials

Administrative Duties

  1. Ensure continuity of care by maintain accurate and concise medical records, participate in clinical case management meetings and communicate with client’s primary care provider and other service providers as required
  2. Participate in chart reviews
  3. Develop, obtain and evaluate diabetes education resources and counseling materials
  4. Assists in the administration of the centre by identifying protocol and procedure improvements and participating in committee work and staff meeting
  5. Provide information and reports as required

Accountability

  1. This position reports to Director of Primary Healthcare Services
  2. The Diabetes Nurse Educator is accountable to the standards of practice of the profession and College Nurses of Ontario
  3. Adhere to all policies and procedures of TAIBU CHC
  4. The Diabetes Nurse Educator is accountable to meet the requirements of any accountability agreement between the Ministry of Health and Long-Term Care and TAIBU CHC

ALL TAIBU CHC employees have some fundamental responsibilities that go beyond the confines of their occupations. These include:

  • Maintaining up-to-date professional skills and knowledge through formal and informal training
  • Acting as an “Ambassador” for TAIBU CHC within and outside the Centre, creating an awareness of TAIBU programs and promoting services offered.
  • Compliance with all mandated legal/regulatory requirements related to working at TAIBU CHC e.g. Employment Standards Act, Occupational Health and Safety, standards of professional conduct, etc.
  • Maintaining awareness of, and compliance with, TAIBU CHC policies and procedures.
  • Maintaining awareness of, and compliance with, funder requirements, policies and procedures, regulations
  • Attending team and staff meetings as required

Qualifications

Academic

  1. Undergraduate degree from a recognized university and/or college
  2. Registered and a member in good standing with the College of Nurses of Ontario
  3. Certified Diabetes Educator (CDE) considered an asset
  4. Certification in adult education or group facilitation considered an asset
  5. Current CPR certification
  6. Demonstrated current knowledge of evidence based best practice and diabetes clinical practice guidelines

Experience

  1. Two to four years experience in clinical counseling, community based diabetes education
  2. Working experience with racialized and marginalized communities is considered an asset
  3. Experience in program development, implementation, monitoring and evaluation
  4. Proficiency in the use of computer and applicable software programs

Personal

  1. An understanding, patient individual sensitive to a high needs priority population
  2. Excellent decision-making, problem-solving, conflict management and time management skills.
  3. Commitment to continuous learning, quality improvement and innovation
  4. Possess an interest in developing new skills relative to identified needs of clients and enhancing current professional practice to complement the agency
  5. Knowledge of community resources, health promotion and social determinants of health and public health policies and their impact on marginalized and racialized communities
  6. Excellent communication skills (written and verbal) and group facilitation skills are an asset
  7. Proven consultation and collaborative skills
  8. Ability to work evenings and occasional weekends
  9. Ability to work independently as well as part of multi-disciplinary team
