About AFHTO The Association of Family Health Teams of Ontario (AFHTO) supports primary care teams across Ontario in delivering high-quality, team-based care. We are launching two QI initiatives across our member organizations:


• Optimizing Adult Vaccination for Respiratory Diseases (influenza, pneumococcal, RSV, COVID-19)


• Adult HPV Awareness and Preventive Health

These projects aim to improve vaccination uptake and awareness through evidence-based interventions, team engagement, and measurable outcomes. Role Overview Reporting to the Director of Health System Data, Research, and Quality Improvement, the QI Coach/Specialist will lead and support participating primary care teams in implementing these initiatives. The Incumbent will serve as a critical link between AFHTO and the primary care teams. The successful candidate will apply QI methodologies, facilitate project planning, and ensure measurable improvements in vaccination rates and awareness. By building staff readiness and optimizing workflows, the QI Coach/Specialist will play a critical role in equipping teams to improve their practice round the subject of the two QI projects. Key Responsibilities


• Project Leadership & Coaching o Guide teams through QI steps: needs assessment, gap analysis, intervention

design, PDSA cycles, and evaluation. o Facilitate coordination meetings, troubleshooting sessions, and progress updates.

o Track, record and provide updates to the teams and AFHTO on activities, deliverables, ensuring timelines, interdependence, and risks are clearly communicated.

o Recommend prioritization of deliverables based on impact, risk, and readiness o Conduct current state mapping and gap analyses for high-impact/high-frequency

workflows to identify risks, opportunities, and areas for improvement, translating findings into actionable change strategies that support safe, standardized adoption

o Deliver hands-on support to guide staff for building strategies to improve practice, reinforce workflows and address real-time adoption challenges


• Data & Process Improvement o Support teams in establishing baseline measures, tracking vaccination rates, and

analyzing outcomes. o Assist with root-cause analysis (e.g., Fishbone, 5 Whys) and process mapping for

vaccination workflows.
• Toolkit & Resource Development

o Apply evidence-based practices and adult learning principles to develop education and QI strategies, tools and templates that promote understanding, retention, and consistent adoption of improvement strategies across disciplines.

o Co-develop, maintain and disseminate QI tools: checklists, tracking sheets, patient education materials, and provider toolkits.

o Ensure resources are reusable, standardized, and easily adopted by multiple clinical areas.

o Harmonize tools across teams and prepare final toolkit for province-wide dissemination.


• Engagement & Communication o Support and promote outreach strategies, patient reminders, and provider prompts. o Prepare reports, lessons learned, and best practices for AFHTO leadership and

conferences. Qualifications


• Degree in health sciences, public health, or related field; QI certification (Lean, Six Sigma, IDEAS) is an asset.


• Minimum 3 years of experience in healthcare quality improvement, and data analysis and interpretation preferably in primary care.


• Strong facilitation and coaching skills; ability to engage multidisciplinary teams.
• Familiarity with vaccination programs, EMR workflows, and preventive health strategies.
• Knowledge of change management principles and strategies to support readiness and

adoption.
• Excellent interpersonal, communication, problem-solving and facilitation skills, with the

ability to engage leaders, interdisciplinary teams, and frontline staff. Working at AFHTO
