Position: Primary Health Physician
Reports to: Director Primary Healthcare Services
Terms: Permanent Full-Time (35 hours/week)

POSITION SUMMARY

TAIBU Community Health Centre provides primary health care and related services for Black populations across the Greater Toronto Area as its priority population and residents of the local community of Malvern. Recognizing that systemic oppression has fostered conditions of ill-health with Black communities, we strive to deliver these services through intersectional, equity based and culturally affirming practices which promote holistic wellness, health education, and prevention.
As part of the Black Focused Interprofessional Primary Care Team, the primary healthcare physician will provide clinical assessments, diagnosis, treatment and follow-up to individual clients, within the scope of their practice. The physician will also be called upon, occasionally, to support health promotion and advocacy initiatives of the Centre. All regulatory and statutory requirements of the profession will be faithfully followed by the physician. Internal accountability procedures will also be completed in a complete and timely manner as required. The physician will work within an anti-racist/anti-oppression framework.

RESPONSIBILITIES

Clinical Services
1. Perform assessment, diagnosis, treatment, health education/counselling, appropriate follow-up and other required medical functions according to the scope of practice and standards of the profession as defined by College of Physicians and Surgeons of Ontario
2. Encounters with clients will generally include:
• A primary health care focus complemented by health promotion, illness prevention and education
• Holistic assessment which assesses the impact of the determinants of health on client outcomes
• Diagnosis
• Treatment
• Referral to specialists, allied health professionals and/or community services as indicated
• Provide medical crisis intervention and triaging
• Follow-up
• Collaborating with other TAIBU programs and services and other outside agencies as needed
• Home visits according to Centre protocols
3. Contribute to the development, implementation, monitoring and evaluation of medical practices, protocols and directives
4. Outbreak surveillance and advisement
5. Share 24-hour on-call duties with other Community Health Centre physicians and as designated

Inter- / Multi-disciplinary practice
1. The Primary Health Care Physician should work across disciplines in order to improve client outcomes
2. Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services for individuals, families and the community
3. Additional duties will be:
• Working with team reviewing outcomes and statistics as required for redirection, changes in clinical practice focus
• Working with the team on networking and outreach as required
4. Attending meetings or sessions representing TAIBU Community Health Centre
5. Attending case conferences as needed
6. Working with the team and the staff on special programs, e.g. diabetes clinic

Administrative Duties
1. Share administrative and clinical maintenance tasks with other centre staff. These may include dealing with pharmaceutical sales representatives, compiling statistical data, updating records, peer review of client records,
2. Vetting, approving, editing and organizing health education materials
3. Participate/lead in/ research initiatives

Accountability
1. This position reports to the Director Primary Healthcare Services
2. The physician has accountability to the standards of practice of the profession and the College of Physicians and Surgeons of Ontario
3. The physician is accountable to meet the requirements of any accountability agreement between Ontario Health and TAIBU CHC

Qualifications

Academic
• An academic preparation as a Medical Physician (an asset: with CCFP credentials) and a medical degree from a recognized university
• A license to practice medicine in Ontario from the College of Physicians and Surgeons of Ontario and a certificate of good standing from same
• Possess or is eligible to obtain required malpractice coverage
• Preference given for a person with three to five years primary care experience; preferably in a community health setting
• Experience in program development, implementation, monitoring and evaluation, an asset
• Proficiency in the use of computers and various software applications

Personal
• An understanding, patient individual sensitive to a high needs priority population
• Enjoys working in a teamwork setting
• Possess an interest in developing new skills relative to identified needs of clients and enhancing current professional practice to complement the agency
• Knowledge of community resources, health promotion and social determinants of health and public health policies and their impact on marginalized and racialized communities
• Excellent communication skills (written and verbal) and group facilitation skills are an asset
• Proven consultation and collaborative skills
• Ability to work evenings and occasional weekends
• Ability to work independently as well as part of multi-disciplinary team
• Interest in supervising/mentoring students
• Possesses a valid driver’s license

Compensation:

$275,538 – 331,974 + Competitive benefits
Expected salary adjustment in the near future – based on Ministry communication
New recruits at TAIBU CHC will be enrolled in the Healthcare of Ontario Pension Plan (HOOPP)

TAIBU is a supported environment with interprofessional care team including allied health professionals

Limited admin and other overhead challenges.

This vacancy is a new position.
