
Primary Counsellor (Temporary)
SurexCARE is a community-based non-profit organization providing support through housing and community supports programs to adults with developmental and intellectual disabilities. We promote social inclusion, active participation in our communities, offer choice, and facilitate independence for those we support.
This is a temporary assignment, lasting approximately 3 months.
Job Description:
Implements the Agency's stated mission and policies and procedures through the provision of direct services to all assigned clients. Assumes responsibility for properly documenting, effectively implementing, and maintaining the planned programs for all assigned clients. Assists and educates clients with regard to daily living skills and acts as an effective member of an interdisciplinary team in the Individual Program Plan process for all assigned clients.
Maintains a professional approach with the family members of clients and provides opportunities for effective involvement between clients and their families. Identifies and organizes activities within individual client programs in accordance with individual need and program focus. Promotes an environment for clients which fosters personal growth and socialization by optimizing community interaction at every opportunity. Carefully observes the physical well being of all assigned clients and communicates, to the Group Living Manager, any concerns in this regard.
Properly documents and administers medication in accordance with policy and procedure.
Arranges for emergency medical treatment for clients, as may be required.
Assumes responsibility for meeting the ongoing clothing and personal needs of assigned clients and assists them in all aspects of personal hygiene and daily living skills.
Ensures a safe, clean and well maintained environment for clients by carrying out basic facilities maintenance and reporting on repair requirements, adhering to all health and safety policies and procedures, and acting on and reporting any dangerous situations regarding client behaviour.
Maintains positive and effective communications with co-workers and other Agency employees and, when representing the Agency, with the public.
Attends all required meetings and training sessions.
Maintains required client and operating records and is responsible for cash on hand for assigned clients and for petty operating expenses.
Responsible for driving clients to various places such as medical appointments, community outings, etc
Requirements:
A two year diploma in DSW or closely related academic discipline, or a Degree in Social Sciences.
Excellent written and verbal communication skills.
Possess basic computer skills (Microsoft Work, use of email, internet navigation).
Physically able to lift a minimum of 30 pounds.
Holds a valid G Class Ontario Driver's license (with a 2 year clean driving record, if applicable.)
