LOFT was chosen to be the Homecare and Community Services provider for CAMH's new Hospital to Home (H2H) program. The standard Service Period for a H2H Program is sixteen (16) weeks in duration, with one Service Bundle to be assigned to the Patient and delivered over the course of this defined timeframe, with Services provided in accordance with the Patient Transitional Care Plan. The H2H program is designed to support adults 55+ facing complex mental health challenges, addiction, dementia and responsive behaviours along with physical health and care needs. This program will focus on individuals lacking access to appropriate supports while living in the community and other social determinants of health. The H2H LOFT Service Team will be working closely with CAMH partners as part of an integrated care model and ensures contact with referred individuals in the community within 24 hours.&nbsp;As part of this mobile team, staff will be travelling across the GTA to meet and work in a variety of environments to support individuals where they are at including in their homes.&nbsp; The Path Home is a 36-unit ALC transitional housing program designed to provide supportive care to individuals living with mental health challenges. Our program aims to create a nurturing environment where clients can transition from acute care settings to a more stable living arrangement. Our program operates in partnership with the Centre for Addiction and Mental Health (CAMH) and the University of Health Network (UHN) hospitals, exclusively accepting referrals from these organizations. Our dedicated nursing staff offers medical monitoring, wound care, and medication management, while clients also have access to primary care services and case management services. Life skills training and community engagement activities empower our clients to live independently and experience meaningful connections. General Office Volunteer - High Intensity (remote) Organization: Right to Live Canada Right to Live Canada | 30+ hrs/week | Volunteer Position------ The Mission Canada is currently facing a human rights crisis of international consequence: poverty-driven disability euthanasia (MAiD). Individuals, blocked by state-enforced poverty, are choosing to end their lives with state assistance due to being unable to access essentials such as medical procedures, supportive housing, or even food. This systemic failure has triggered a United Nations investigation, criticism from the Canadian Human Rights Commission, and global condemnation.Right to Live Canada is engineering the only viable, system-level alternative to this crisis -- Social Brokerage. By creating a pipeline between sources of raw capital and specific one-time support plans ("Stability Mandates") for individuals in the process of applying for MAiD, the organization creates the "viable alternative" that the system itself mandates its assessors to find but does not currently provide. This represents the world's first market-based solution to poverty-driven euthanasia and the creation of a social impact credit system which exchanges philanthropic capital for verified, life-affirming outcomes, akin to carbon credits.Right to Live Canada is currently pre-pilot.------ The Opportunity Right to Live Canada is a start-up non-profit solving economic euthanasia. We are in need of high intensity, committed volunteers who are ready and willing to put in the hard work to &nbsp;advance our work. This means a volunteer load of 30+ hours per week, for a duration up to the volunteer. The only common skill requirement is the ability to work in a remote office environment (Slack, Google Workspace). We will assign you a relevant role based on your resume and skill background.This opportunity would be ideal for those who have the opportunity for sufficient availability (summer break, unemployment, retired, etc) and want utilize their time in an impactful and meaningful way.
