Primary CONNECT (Collaborative Navigation Network for Connected Treatment) is a new comprehensive

Interprofessional Primary Care Team (IPCT) that is a collaborative effort between two CHCs (Parkdale Queen West CHC & South Riverdale CHC), and CAMH’s specialist mental health and substance use care/ addictions services, and through CAMH’s Shkaabe Makwa, will provide culturally responsive supports for First Nations, Inuit and Metis clients. The Primary CONNECT collaborative enables the three organizations to create a service that supports a high-need under-serviced population with mental health and substance use care/addictions needs in the downtown core and East Toronto. This service is supported by the provincial government’s Expanding and Enhancing IPC Teams initiative, focused on access and attachment for clients/patients with poorer health outcomes/health status.

We are seeking an experienced Physician who will be responsible for providing holistic health care to individuals, families and groups. The Physician may act as a supervisor to health care students and participate in research and quality improvement activities. The Physician will be accountable for reporting to the Clinical Manager, Integrated Care.

General Responsibilities:

• Provides holistic health care to clients of all ages.
• Provides brief counselling and support to clients with mental health concerns, emotional problems or other difficulties and makes referrals to internal or external programs and services.
• Provides on-call coverage on a rotating shared basis with other physicians to ensure medical care can be accessed 24 hours per day throughout the entire year.
• Participates in community outreach, client education and prevention programs.
• Identifies trends or issues related to the health status of priority groups and uses this information in planning and advocacy work.
• Provides leadership, supervision and guidance in the general organization and operation of the clinic’s medical activities.
• Maintains clear, accurate and timely written documentation including client charts, requisitions, reports, encounters and other information as required by the health center, Ontario Health, and the College of Physicians & Surgeons of Ontario.
• Participates in the design, implementation and evaluation of research studies in consultation with the Management team and other staff.
• Acts as a supervisor and consultant to health care students and participates in the learning of students from other disciplines.

Qualifications:

• Member in good standing of the College of Physicians and Surgeons of Ontario (CPSO) and the College of Family Physicians of Canada.
• Experience working with people who have history of substance use and /or mental health issues.
• Experience working with clients who identify as 2SLGBTQ+.
• Experience working with First Nations, Inuit and Métis clients and communities.
• Demonstrated ability to work independently and interdependently in a multi-disciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
• Demonstrated understanding of the role and responsibility of staff of Parkdale Queen West in addressing the diverse and complex health and social needs of community members, particularly those experiencing poverty and discrimination.
• Strong oral and written communication skills.
• Excellent decision-making and time management skills.
• Understanding of anti-oppression principles and frameworks.
• Embraces a harm reduction approach to care.
• High level of comfort with diverse populations; demonstrated ability and comfort in working with people who use drugs and people with

Compensation: $182.40, including 6% in lieu of vacation
