The Homelessness and Addiction Recovery and Treatment (HART) Hub Physician provides comprehensive primary care which emphasizes accessibility, health promotion, illness prevention, harm reduction, and continuity of care for our clients, including those experiencing homelessness, those with complex substance use, and those with mental health needs and other structural vulnerabilities in our large urban setting. Our clients include refugees and those with and without OHIP, comprising adults, youth, and families with young children. The Physician is part of the HART Hub services which work closely with the mental health, case management, outreach, and health promotion teams to ensure cross-team and cross-site collaboration. The Physician will work with other members of the HART Hub team to contribute to the continuing growth and development of the HART Hub program.

Reporting to HART Hub management, the Physician will also participate in HART Hub community partnerships led by the Centre, including with CAMH, Parkdale Activity and Recreation Centre (PARC), The Neighbourhood Group, and others, sharing knowledge and resources and providing offsite care as required.

The physician functions both independently and as part of a multi-disciplinary HART Hub team. The work will be based at the Queen West site (168 Bathurst St.) and may include some offsite clinic locations and outreach.

Reporting Manager: Clinical Manager, Integrated Care (Queen West Site)

Specific responsibilities include, but are not limited to:

  • Providing primary care directly, emphasizing accessibility, health promotion, illness prevention, harm reduction, and continuity of care.
  • Conducting comprehensive assessment and history taking, diagnosing, and developing a care plan in a client-centred manner with HART Hub clients
  • Working interactively with nurse practitioners and registered nurses, providing consultation to nurses or other staff, and by assisting in development of medical or clinical protocols.
  • Providing on-call coverage on a rotating shared basis with other physicians to ensure that medical care can be accessed 24 hours per day throughout the entire year.
  • Participating in quality improvement activities.
  • Acting as a preceptor to medical students and residents and participating in the learning of students from other disciplines.
  • Providing leadership in the general organization and operation of the clinic’s medical activities.

Qualifications:

  • Member in good standing of the College of Physicians and Surgeons of Ontario and the College of Family Physicians of Canada.
  • Experience and interest in working with clients who identify as 2SLGBTQ+.
  • Experience and interest in working with people who have a history of substance use and /or mental health issues.
  • Demonstrated ability to work independently and interdependently in a multi-disciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
  • Demonstrated understanding of the role and responsibility of staff of Parkdale Queen West in addressing the diverse and complex health and social needs of community members, particularly those experiencing poverty and discrimination.
  • Willingness to participate in Parkdale Queen West’s commitment to becoming discrimination-free Health Centre.
  • Fluency in languages other than English is an asset.

Compensation: $291,461- $331,974 per annum, plus Group Benefits and HOOPP (Health Care of Ontario Pension Plan).
