The ROLE

As a ROCK Pathway Support Counsellor, you will help strengthen continuity between Home-Based Intervention and Live-In Treatment programs. You will support transitions between levels of care, helping youth and families move smoothly through services while improving overall client flow and access to care.

The Pathway Support Counsellor is part of the Pathways for Family Intervention and Treatment (PFIT) team and plays an important role in supporting youth and families as they transition between Home-Based Intervention and Live-In Treatment programs. This role helps ensure continuity of care, coordinated service planning, and smooth transitions between levels of service while improving overall access to care and client flow.

This is an in-person position working three to four evenings per week (until 8:00–9:00 p.m.) in both Live-In Treatment and Home-Based Intervention (in clients’ homes). The Pathway Support Counsellor will also participate in the PFIT Consultation Team, contributing to client consultations, case reviews, and integrated care planning.

This is a newly created, full-time contract position with an end date of March 31, 2028

Key Responsibilities:

  • Work flexible evening shifts in both Live-In Treatment and Home-Based Intervention settings.
  • Act as the primary Counsellor for designated youth in the PFIT follow-up pathway.
  • Assist in the development, implementation, and monitoring of client treatment plans and plans of care.
  • Support transitions between Live-In Treatment, Home-Based Intervention, and community services.
  • Collaborate with youth, families/caregivers, and the PFIT team to develop coordinated, client-centered care plans.
  • Consult and collaborate with interdisciplinary teams and community service providers.
  • Provide culturally responsive services that reflect the diverse needs of families.
  • Deliver services in a variety of settings including homes, Live-In Treatment programs, schools, and the community.
  • Use a variety of therapeutic interventions including individual, family, and parent-focused support.
  • Provide in-home support including behaviour management strategies, emotion regulation support, risk assessment and safety planning, parent coaching, life skills support, and connections to community resources.
  • Complete administrative duties including documentation, data entry, monthly statistics, and reporting requirements.
  • Follow all agency policies and procedures related to client safety, documentation, and health and safety.
  • Provide supervision to students as required.
  • Other related duties as assigned.

Who we’re looking for:

Education

  • Diploma or Degree in Child and Youth Work, Social Work, or a related field from a recognized university.
  • Eligibility for registration with the Ontario College of Social Workers and Social Service Workers and/or the College of Registered Psychotherapists of Ontario (CRPO) is considered an asset.

Experience

  • Minimum of three (3) years of clinical experience working with children, youth, and families.
  • Experience providing individual, family, and group interventions.
  • Experience working in community-based and/or residential treatment settings is preferred.

Skills

  • Strong clinical, organizational, and time management skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong collaboration and coordination skills.
  • Knowledge of the Child, Youth and Family Services Act and children’s mental health services.
  • Ability to work flexible hours, including evenings.
