Salary: $61.11 - $74.29/hour (up to maximum of 35 hours per week)
BQWCHC, in partnership with the Primary Care Network of the Hastings Prince Edward Ontario Health Team (HPE OHT), is pleased to announce the establishment of the Belleville Neighbourhood Clinic and to invite Nurse Practitioners interested in practicing in a shared care model to participate.
The Neighbourhood Clinic is affiliated with the Belleville and Quinte West Community Health Centre (BQWCHC) but will operate in a separate location providing Primary Care services to better serve our community. The Clinic is part of the HPE OHT’s strategy to increase access and attachment to primary health care in HPE.
The Opportunity
We are seeking Nurse Practitioners committed to client-centered care who are passionate about innovative thinking and collaborative teamwork to provide primary health care to a defined patient roster. The Belleville Neighbourhood Clinic is developing a novel shared-care model where patients are rostered to the Clinic rather than an individual provider. Direct patient care and inbox responsibilities will be shared among a team of interested physicians and NPs, with robust support from a community-based interdisciplinary health care team comprised of RPN’s, RN’s, administrative staff and allied health professionals. The Clinic will deliver comprehensive primary health care services, including assessment, diagnosis, treatment, prevention, education, rehabilitation and/or therapeutic support, to clients of all ages for optimal health outcomes.

QUALIFICATIONS
- Registration in good standing as a Registered Nurse with the College of Nurses of Ontario in the Extended Class (EC)
- Experience as a Nurse Practitioner, with preference to those with experience in primary health care or in a community setting
- Demonstrated problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment
- Commitment to working within and optimizing the interdisciplinary team
- This role includes primarily Monday to Friday daytime clinic hours, with some evenings, to better accommodate patients’ needs and improve access to care.
