Nurse Practitioner, HART Hub

The Nurse Practitioner delivers primary care services with a focus on addictions and mental health treatment and recovery through a variety of methods including direct client-care; health promotion and education; and individualized treatment plans, including medication management, counseling, and referrals to other healthcare providers. The Nurse Practitioner works within a trauma-informed, harm reduction framework that promotes the mission and values of RPCHC.
