Job Description – Nurse Practitioner (full-time, permanent) Position Summary

The Nurse Practitioner (NP) is an integral part of a multi-disciplinary team of physicians, social workers, dietitians, RNs, pharmacists, and others clinicians, whose role will increase access to primary care and enhance the quality of life for patients. The NP will practice within his/her full scope of practice as outlined in the Standards of Practice by the College of Nurses of Ontario.

Roles and Responsibilities

Patient Care

  • Provide primary health care services within the defined Primary Health Care Nurse Practitioner extended class scope of practice and Bill 179 (Regulated Health Professions Statute Law Amendment Act, 2009) including assessment, diagnosis, treatment, prescribing and dispensing of controlled drugs and substances, as per Controlled Drug and Substances Act (CDSA) and management of health and illness in primary care.
  • Perform patient health assessments and health promotion activities
  • Monitor the ongoing care of patients with chronic conditions
  • Screen for the presence of chronic illnesses
  • Conduct examinations using an age/sex specific list of recommended preventative interventions and risk assessments
  • Diagnose and treat acute/episodic illnesses and injuries
  • Provide phone consultation/triage
  • Conduct home visits for housebound patients
  • Formulate and communicate medical diagnoses and disorders
  • Order and interpret diagnostic tests and procedures (x-ray, CT, MRI, ultrasound, BMD, PFT, ECG, stress test)
  • Prescribes medications and treatments within the scope of the NP as outlined by the College of Nurses of Ontario

Referral/Consultation/Collaboration

  • Consult with a physician as per the Primary Health Care Nurse Practitioner Standards of Practice as defined by the College of Nurses of Ontario and/or as needed.
  • Work with other team members as required
  • Arrange for external specialist consultations
  • Communicate with hospital, community agency staff and other interdisciplinary health providers to ensure smooth care transitions

Education and Advocacy

  • Provide group health education (birth control, hypertension, menopause, smoking cessation, osteoporosis, sexuality, breastfeeding and parenting)
  • Provide in-service education for FHT
  • Teach family medicine residents, nursing students and other team members

Administration

  • Consistently document in the patient’s electronic medical record (EMR)
  • Maintain a preventative health monitoring program using EMR reminder functionality
  • Arrange follow-up appointments as necessary
  • Participate in the development and evaluation of policies, protocols and procedures

Other activities may be assigned in collaboration with the inter-professional team

Skills, Knowledge Requirements

General

  • Leadership, critical thinking, organizational and communication skills are required
  • Expertise in developing and maintaining excellent working relationships with all member of the health care team and partner organizations
  • Ability to work in a collaborative practice model
  • Strong health assessment/clinical skills
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment
  • Ability to maintain confidentiality and impartiality
  • Capacity to adapt quickly to a fast paced, dynamic work environment and be self-motivated
  • High degree of accuracy and attention to detail
  • Expertise in gender-affirming care an asset
  • Expertise in women’s health including IUD insertions and asset

Computer Skills

  • Familiarity with a Mac environment
  • Experience with electronic medical records (preference Telus/Practice Solutions)
  • Knowledge of and working experience with external computer systems (i.e. cOntario)

Language Skills

  • English required. French not required but would be an asset.

Education and Specific Job Requirements

  • Required

o Bachelor’s Degree in Nursing

o Registration with the College of Nurses of Ontario as a RN-EC (extended class)

o Current membership in Nurse Practitioners’ Association of Ontario (NPAO)

o Specialty Certificate - NP-Primary Health Care

  • Considered an Asset

o Master’s Degree

Relevant Experience

  • Two-Three (2-3) years’ primary care experience as a NP
  • Community health nursing in a primary care an asset
  • Teaching experience an asset

Reporting Structure

The NP reports to the FHT Executive Director for day-to-day operations. Professional guidance and support provided by site physicians.

Job Specification

Location: 790 Bay St., Toronto, Ontario

Hours of work: 40 hours per week

Note: Expectation is for this position to be on site 80%, 1 day work from home (this is subject to change at any time by the organization)

Status: Full-time Permanent

Pay Range: $113,811 - $125,477

Benefits: 22.5% (used for: statutory deductions, group insurance plan & Hospitals of Ontario

Pension Plan - HOOPP)

Annual Vacation Days: 3 weeks (4 weeks after year 1)

Annual Professional Education Dollars: $1500

Annual Education Days: 10 days

Annual Sick/Personal Emergency Leave: 10 days
