Background: Flemingdon Health Centre (FHC) is a not-for-profit Community Health Centre serving Flemingdon Park, Fairview and Thorncliffe Park. Guided by a vision of Strong Healthy Communities, FHC provides health and community services grounded in health equity, inclusion, community engagement, accountability, excellence and collaboration.
Position Summary: FHC is hiring for a Nurse Practitioner to be deployed and work at Unify. This is part of an exciting initiative where FHC is acting as a lead organization to support the delivery of primary care services. The Nurse Practitioner will be an integral member of the interdisciplinary team, providing comprehensive, client-centred primary care. Working to full scope of practice and in accordance with College of Nurses of Ontario standards, the Nurse Practitioner provides assessment, diagnosis, treatment, health promotion, education and referrals while helping reduce barriers to care and address the social determinants of health.
Deployment Role – Employment Model: While the Nurse Practitioner will be hired and remain employed by FHC, the incumbent will work as part of the Unify care team and follow Unify’s day-to-day site operations, schedules and clinical workflows. Day-to-day direction is provided on site at Unify, while formal reporting and accountability remain with FHC.
Responsibilities Include: Direct Client Care:
Provide comprehensive primary care, including assessment, diagnosis, treatment plans, follow-up, referrals, education and therapeutic support within NP scope of practice.
Order, interpret and follow up on appropriate screening, diagnostic and laboratory tests; prescribe designated drugs as authorized by the CNO.
Maintain accurate client records and coordinate care with health teams, physicians, specialists and community partners.
Support health promotion, chronic disease prevention and quality improvement initiatives.
Health Promotion and Education:
Deliver health teaching, group programs and presentations that align with the stated mission, goals and community priorities.
Identify priority populations and contribute to community development initiatives in collaboration with partners and practitioners.
Organizational Responsibilities:
Participate in team meetings or other working groups as requested.
Consult with physicians and interdisciplinary colleagues in accordance with CNO guidelines.
Participate in annual planning, implementation and evaluation of service goals.
Maintain knowledge of communities, services and resources to support appropriate referrals.
Comply with relevant legislation, policies and privacy requirements.
Support students, volunteers and staff learning as required.
Qualifications, Skills, and Proficiencies:
Registered Nurse in the Extended Class with the College of Nurses of Ontario.
Baccalaureate in Nursing and completion of a recognized Primary Care Nurse Practitioner program.
Two to three years of primary care or community nursing experience is an asset.
Experience with chronic disease management, health promotion, marginalized communities and clients with complex needs.
Strong commitment to anti-racism, anti-oppression, health equity and client-centred care.
Excellent communication, collaboration and organizational skills; able to work independently and as part of an interdisciplinary team.
Ability to work flexible hours, including evenings and weekends.
Additional languages, preferably Spanish or Arabic, and EMR/Practice Solutions experience are assets.
Working Conditions & Physical Requirements:
Incumbent will be expected to work some evening and/or weekend hours based on project
demands.
