Location:

Consistent with Scott Mission wide policies the Health Services Manager will work out of Scott Mission locations based on need and organizational demand; homebase will be 346 Spadina.

The Scott Mission was founded in 1941 by Rev. Morris and Mrs. Annie Zeidman as a non-denominational Christian organization. Each year, it offers practical, emotional, and spiritual support to thousands of people who are experiencing poverty and homelessness in the City of Toronto. The Mission is currently implementing a transformational, five-year strategic plan with the following objectives: increasing capacity and enhancing basic needs services, deepening clients’ transformational impact, multiplying partnerships, and offering spiritual leadership to Canadians.

The Mission operates with a dedicated staff at four locations: 502 Spadina Avenue Toronto, its Family Centre in East York, the Collingwood Retreat Centre, and the Scott Mission Summer Camp in Caledon.

Position Summary: As part of a holistic, Christ-centered vision for transforming lives, Scott Mission has launched a Health Clinic within it’s Shelter located at 346 Spadina Ave in Toronto to address the complex health needs of Shelter residents and community members. The Nurse Practitioner will provide accessible, high-quality care that addresses the complex health needs of the shelter residents and community members served by Scott Mission’s Health Clinic. The Nurse Practitioner will work collaboratively with a multidisciplinary team, including community partners and the staff from Scott Mission’s Shelter, Respite, Welcome Centre and Transformational Ministry programs, to support the strategic ministry direction as articulated by Scott Mission Executive and the Chief Ministry Officer.

Responsibilities:

  1. Client Care
    • Provide direct nursing care to Scott Mission Shelter residents and community members as needed.
      • Develop and implement individualized care plans and provide high-quality medical procedures as necessary for patients.
      • Ensure excellence in client care and follow up in close collaboration with Scott Mission Case Workers, Intake Advocacy Associates, and Shelter Associates.
      • Coordinate referral of patients directly to specialists outside of Scott Mission services.
      • Promote patient education and advocacy.
      • Work collaboratively with Scott Mission Health Services staff and community partners to ensure best possible care for patients.
  2. Leadership
    • Cultivate a vibrant faith and godly character and conduct that is informed by the Vision, Mission, Statement of Faith, and Marks of Scott Mission.
      • Pray regularly for Scott Mission community members, staff, and partners.
      • Attend all required Ministry team meetings, and participate in Scott Mission wide activities and initiatives, including All Staff meetings and retreats.
      • Ensure clear and consistent communication vertically and cross-functionally and foster a culture of transparency and support across all functions within the Mission and directly as part of the Ministry team.
      • Liaise, host, and follow up with medical professionals and other community partners who are providing health services to Scott Mission residents or guests.
  3. Administration
    • Work collaboratively with Health Services staff, Case Workers, and Shelter staff, in the development and enhancement of policies and procedures for Health Services at Scott Mission.
    • Ensure compliance with all healthcare regulations and standards, include TSSS standards, and implement program enhancements based on audits.
    • Collaborate with Scott Mission’s Health & Safety Manager and Shelter staff in the event of an outbreak, and during IPAC and other audits.
    • Effectively manage an annual budget that demonstrates respectful and prudent stewardship of resources.

Required Experience and Education

  • In full agreement with the Statement of Faith, Vision, Mission, and Marks of Scott Mission.
  • Current registration with the College of Nurses of Ontario Extended Class as a Nurse Practitioner and entitled to practice with no restrictions.
  • Proven experience serving diverse and vulnerable urban populations, with demonstrated understanding of issues, including chronic homelessness, poverty, complex mental health issues, and addiction.
  • Proven ability to provide humble, servant leadership to engaged, committed, and high performing teams in a multi-disciplinary health care setting including team development and performance optimization.
  • Self-starter and result-oriented, excellent problem solver, team player.
  • Excellent skills in verbal and written communication, active listening, conflict resolution, and crisis prevention, with the ability to work with people of diverse backgrounds.
  • Strong administrative skills, with demonstrated ability to multitask, plan and execute objectives
  • Understanding of healthcare and PHIPA Act guidelines; familiarity with case management, Shelter standards is an asset.
  • Computer literacy: Windows, Office365, EMR systems
  • Able to work with people of diverse backgrounds. Ability to speak other languages is an asset.
  • Able to work flexibly, including evenings and weekends, as situations demand.
  • Valid Driver’s license.
