
Manager, Substance Use and Community Programs
The Inner-City Family Health Team is a multidisciplinary group dedicated to improving health equity and reducing barriers to care for individuals who have experienced homelessness or housing instability. This clinic offers specialized care for those with complex needs, including physical, mental health challenges, intellectual disabilities, substance use and complex behaviours.
This position will supervise and support the following programs:
- SubHub: Substance Use Care Program
- Community Primary Care Connection
- Here to Help (Community Crisis Team, Co-Lead by Gerstein Centre).
The ideal candidate will have expertise in community engagement, harm reduction approaches, quality improvement and human services. We are looking for someone with a strong vision for client care, social justice and organizational wellness who can support the growth and development of our clinic and teams.
Reporting to the Executive Director, the Manager of Community Programs will contribute to the management of the organization and ensure that services are delivered by the team using a holistic, patient-centred model of care, with an emphasis on prevention, client engagement, harm reduction, and health promotion.
ROLES AND RESPONSIBILITIES:
- Oversees and directs the daily operations of the partnership programs, providing leadership and supervision to 10 employees in a multidisciplinary team, including social workers, nurses, case managers, and other healthcare professionals.
- Coordinates or provides profession-specific clinical support. Support the goals and growth of all staff members by conducting regular performance reviews, coaching, performance management, and providing going feedback to staff.
- Serves as a resource person for staff in the provision of client-centred care. Evaluates the quality of care and makes recommendations for improvement.
- Provides strategic advice and recommendations to the ED on program operations, organizational wellness, partnerships, advocacy and patient care
- Works with ICHA (physician partner) management, ICHA physicians, and FHT clinical and administrative staff to ensure operational efficiency and effectiveness in delivering collaborative care at the clinic, partnering sites and specialized programs such as the Substance Use Hub.
- Maintain and develop strategic partnerships with other community and health services
- Supports the development of health promotion and chronic disease management programs and services in collaboration with all multidisciplinary team members to advance these initiatives.
- Leads Quality Improvement initiatives, data collection and reporting to enhance patient outcomes and satisfaction.
- Leads development of equity practices and ensures policies reflect current practice.
QUALIFICATIONS/OTHER CONSIDERATIONS:
- Minimum 2 years proving leadership and direct supervision, or equivalent experience
- Knowledge and experience of training and supervision within an interdisciplinary team structure
- Minimum of 5 years of experience working with individuals who are marginalized by their social location for reasons including homelessness, poverty, gender identity, sexual orientation, and/or residency status using trauma-informed and anti-oppressive practices.
- Experience working with individuals who are experiencing challenges with their mental health and/or using substances.
- Management in a unionized environment preferred.
- Has proven success with inspiring teams and the ability to mentor, coach and facilitate positive change and innovation; demonstrated ability to motivate others.
- Ability to work with quality improvement methods, including data collection and evaluation.
- An understanding of program development, particularly in the areas of mental health, substance use, harm reduction, adult health, and care of marginalized populations, including homeless individuals.
- Demonstrated ability to de-escalate conflict an asset, and ability to manage stressful situations.
- Knowledge of and demonstrated commitment to principles of harm reduction, injection drug use and associated health issues.
- Knowledge of principles of anti-racism, anti-oppression, and social justice
- Fosters collaborative working relationships within the inter-professional team which optimizes patient care and develops a healthy workplace.
Salary: $80,765 - $91,535. Eligible for HOOPP Pension and full benefits after successfully completing the probationary period.
