
Housing Follow-Up Support Field Supervisor
Christie Ossington Neighbourhood Centre (CONC) is a multi-service community agency committed to supporting individuals facing homelessness, poverty, and systemic barriers. We are currently seeking a Full-Time, Permanent Housing Follow-Up Support Field Supervisor to lead the delivery of mobile case management services for individuals transitioning from homelessness to long-term housing.
This role ensures consistent, client-centered service delivery by supervising frontline staff, coordinating referrals, managing a reduced client caseload, and supporting program evaluation and outcomes tracking. The Supervisor plays a key leadership role in helping clients stabilize their housing, improve health and wellness, strengthen life skills, and build long-term independence.
Key Responsibilities
Client Support & Casework
- Complete client intakes, conduct home visits, and provide crisis intervention and supportive counseling.
- Develop and monitor individualized case plans to support long-term housing stability.
- Advocate and liaise with landlords, OW, ODSP, COHB, healthcare providers, and community partners.
- Provide life-skills coaching in budgeting, tenant rights, household maintenance, and daily living.
- Maintain accurate and timely case notes and documentation using Salesforce.
Team Supervision & Training
- Provide day-to-day supervision, support recruitment efforts, and participate in interviews.
- Lead onboarding, mentorship, field support, and ongoing training for staff.
- Promote a positive, inclusive team environment based on anti-oppressive principles.
- Support performance management and progressive discipline processes as required.
Program Leadership
- Build and maintain partnerships with housing, healthcare, settlement, and community agencies.
- Facilitate life-skills workshops that promote independence and community integration.
- Lead weekly case conferences to support collaborative service planning.
- Track program outcomes and assist with reporting requirements, including TGRIP.
- Ensure adherence to Hostel Standards, agency policies, and professional conduct guidelines.
Qualifications
- Bachelor’s Degree in Social Work, Psychology, or related human services field.
- 3–5 years of program management experience.
- At least 5 years working with vulnerable adults experiencing homelessness, mental health issues, substance use, or related challenges.
- Minimum 2 years of frontline supervisory experience OR equivalent leadership experience with strong service excellence record.
- Knowledge of housing stability models, harm reduction, trauma-informed care, and community development.
- Strong leadership, critical thinking, and conflict resolution skills.
- Experience working within a unionized environment considered an asset.
- Proficiency in documentation, outcome tracking, and case management systems.
- Standard First Aid & CPR certification required (or willingness to obtain).
- Ability to work flexible hours as required.
