The Homelessness and Addiction Recovery Treatment (HART) Hub demonstration project is a multi-disciplinary, collaborative initiative between the Parkdale Queen West Community Health Centre, and 11 partnered organizations. Through a “hub and spoke” model we will provide culturally safe and trauma informed services to individuals with complex health and social care needs. Services will include primary care and substance use care (continuum of treatment, recovery and harm reduction); mental health care and support services; supportive housing including shelter and transition beds; and social services including peer support, Canadase management, and employment supports.

Job Summary:

As part of the Seeking Safety Group, you will contribute to the facilitation of an evidence-based, present-focused counseling program designed to help individuals attain safety from trauma and substance use. The Seeking Safety approach allows participants to explore their relationship to substances while focusing on safety and healing, without requiring individuals to revisit their past trauma. This program is integral to the first stage of trauma recovery, offering individuals the tools to create safety within themselves and their environment.

Key Responsibilities:

  • Ensure the effective and compassionate delivery of services in the Drop-In space, providing direct personal support and managing safety for all PARC members. You will oversee activities such as running small group programs, recreational activities, and responding to crisis situations.
  • Provide trauma-informed, anti-oppressive engagement for all Drop-In members. Facilitate a direct referral pathway to PQWCHC for healthcare and other essential services.
  • Co-facilitate the Seeking Safety program, supporting participants in addressing trauma and substance use through a present-focused, safety-envisioning approach. Ensure participants feel supported without the need to rehash past traumatic experiences.
  • Collaborate with other PARC staff, volunteers, and community partners to ensure holistic support for all members. Work to address systemic barriers and contribute to public education and awareness.
  • Maintain accurate records of interventions, member concerns, safety issues, and other relevant data. Participate in daily team debriefing sessions, offering critical reflection and analysis of ongoing challenges and successes.
  • Assist in the development of community resources and strategies to address service gaps, ensuring a comprehensive support network for members in need.

Qualifications:

  • Minimum of 2 years of experience in harm reduction or related fields, particularly in drop-in settings serving marginalized adults, psychiatric survivors, and individuals with substance use issues. Experience with crisis intervention, trauma-informed care, and advocacy is required.
  • Experience in overdose prevention and naloxone administration, including the ability to train others in these practices.
  • Post-secondary education in a social service-related field or an equivalent combination of education and direct experience in mental health, addiction, and social services.
  • Strong communication, organizational, and documentation skills. Ability to work effectively under pressure and in a team-oriented, high-stress environment.
  • Experience in navigating complex systems such as housing, legal support, and food security. Strong commitment to addressing systemic barriers for marginalized populations.
  • Proven skills in group facilitation, conflict mediation, and providing support to individuals in crisis.
  • Demonstrated understanding of and commitment to anti-racist, anti-oppressive frameworks in both practice and policy

Work Hours:

Employees work 37.5 hours per week, ranging between the hours of 8:30am and 6:00pm.

This role requires work to commence either Wednesday to Sunday, Tuesday to Saturday, or Sunday to Thursday.
