Position Summary

AFHTO is seeking a strategic, systems-oriented and highly collaborative leader to take on the newly created role of Director, Strategy, Policy and System Transformation. As a key strategic partner and right-hand to the CEO, this role will drive AFHTO’s work in primary care system transformation by advancing policy implementation, supporting members through operational complexity, and aligning internal strategy with broader system change.

This is a high-impact, non-advocacy role grounded in the practical realities of interdisciplinary primary care teams and the policies, contracts, and funding models that shape them. The Director will lead the translation of member and operational insights into systems-level responses, guiding AFHTO’s approach to strategic engagement with government and health system stakeholders, and helping shape the future of team-based care in Ontario.

In addition to their system strategy and policy leadership, the Director will play a critical role in stewarding AFHTO’s internal governance evolution and working closely with the CEO to ensure that the Board, Committees, and Councils are effective, strategically aligned, and responsive to member and system priorities. This role also provides strategic coherence to AFHTO’s Governance and Leadership Program for members, ensuring it reflects both the realities of team-based care and the expectations of an evolving health system.

Key Responsibilities

Strategy & System Transformation


• Serve as a core strategic advisor to the CEO on system transformation priorities, especially related to Ontario’s evolving primary care landscape.

• Ensure AFHTO’s work aligns with a clear and future-oriented strategic vision for high- performing, team-based care.


• Directing annual business planning, strategic planning and reporting to move the strategic objectives of the organization forward in meaningful ways.


• Identify connections between member challenges and broader system-level opportunities; develop actionable responses rooted in evidence, contracts, and policy.


• Translate high-level strategy into practical frameworks, reporting and cross-organizational alignment.

Policy Analysis and Implementation


• Lead or guide analysis of policies, contracts, and funding models affecting interdisciplinary primary care teams.


• Translate evolving government direction into actionable supports for members (e.g., guidance on implementing new policies or navigating existing frameworks).


• Collaborate with members and partners to surface issues, identify enablers, and co- develop practical, policy-informed solutions.


• Ensure internal teams are equipped to understand and respond to policy shifts that affect AFHTO’s members.

Member and Stakeholder Engagement


• Build and maintain strong, trust-based relationships with AFHTO members to understand their needs and bring those perspectives to system tables.


• Engage with government, Ontario Health, and other system stakeholders to ensure AFHTO is seen as a credible, solutions-focused contributor.


• Facilitate a two-way flow of information and insight between members and system partners to support evidence-informed, responsive policy development.


• Represent AFHTO in high-level engagements, system strategy discussions, and policy co- design opportunities.

Strategic Oversight: Governance and Leadership


• Serve as a key partner to the CEO in shaping and stewarding AFHTO’s governance architecture. As well as ensuring that the Board, Committees, and Councils are structured and supported to advance the organization’s mission and strategy.


• Leverage AFHTO’s governance structures as core mechanisms for member engagement, issue identification, and strategic insight.

• Shape, in partnership with the CEO, the Governance and Leadership Program for members. As well as ensuring the development and delivery of tools, resources, and capacity-building supports that enable strong governance within primary care teams.


• Provide strategic guidance to the Director of Member Partnerships and Program Excellence responsible for the Governance and Leadership Program including day- to-day program delivery, ensuring coherence with AFHTO’s broader strategy and system goals.

Internal Leadership and Collaboration


• Collaborate across the leadership team to integrate strategy, policy, governance, and member support functions.


• Foster alignment across internal initiatives to ensure coherence with AFHTO’s external positioning and strategic priorities.


• Support team members in understanding how policy, contracts, and system direction influence their work and member-facing activities.


• Represent AFHTO in high-level discussions, strategy sessions, or forums alongside (or on behalf of) the CEO.

Qualifications and Experience

Required:


• 8+ years of progressive experience in health system strategy, policy, and implementation preferably in a member-based organization.


• Deep knowledge of Ontario’s primary care landscape, including interdisciplinary team models, policy, funding, and governance structures.


• Experience analyzing and translating policy or contractual frameworks into actionable supports for frontline organizations.


• Demonstrated ability to operate at both strategic and systems levels, while understanding operational and frontline implications.


• Proven ability to build trust-based relationships across sectors, including with members, health system leaders, and internal teams.


• Excellent communication, facilitation, and problem-solving skills.


• Comfortable working in ambiguity and leading with autonomy in a fast-paced, evolving and entrepreneurial environment.


• Leadership experience guiding cross-functional teams, governance bodies, or multi- stakeholder initiatives with strategic influence.

Preferred:


• Familiarity with AFHTO’s member organizations or similar health system associations.


• Experience supporting or advising organizational governance structures (Boards, Committees, etc.).


• Experience with member-facing leadership development or capacity-building initiatives.


• Background in non-profit, association, or member-based environments.
