Position Title: Director of Integrated Care

Work Hours / Status: 35 Hour Work Week - Full Time Permanent

Salary: $132,000 – $142,596

Position Reports To: Chief Operations Officer

Location: Remote

Summary – Purpose of the Job

The Director of Integrated Care is a part of The Indigenous Primary Health Care Council (IPHCC)’s senior leadership team and is responsible for providing consistent strategic and organizational support to teams by planning, managing, and evaluating activities and outcomes for the delivery of high-quality inter-professional health care programs and services for optimal individual and community health outcomes.

Key Duties/Responsibilities

  • Provide strategic leadership, management, and operation of the clinical and community programs and services.
  • Plan, develop, and implement initiatives to support the development and delivery of programs and services within the context of Indigenous Health Models and well-being and from a health equity approach.
  • Work with Program Managers and their teams to promote leading patient delivery models and implement best practices related to high quality Indigenous and community care.
  • Lead the development, monitoring, and analysis of continuous quality improvement and clinical initiatives.
  • Handle difficult member situations or crises that require advanced training.
  • Develop evaluation methods for existing programs and reviews recommended improvements made by the team for implementation.
  • Develop and implement quality improvement initiatives, monitoring and measuring performance including primary responsibility for oversight of Quality Improvement Plans.
  • Lead and manage team using a servant leadership model, fostering collaboration and teamwork within the organization.
  • Work with the Human Resources Department to recruit and retain highly skilled staff within the organization.
  • Assist in the leadership of Accreditation.
  • Attend board meetings and provide information reports and presentations as required.
  • Develop and maintain community partnerships that increase the capacity of services and programs that are available for IPHCC members and clients.
  • Cultivate partnerships that increase accessibility of IPHCC members and community services for IPHCC priority populations.
  • Participate in IPHCC annual budget development process.
  • Provide ongoing business continuity planning and anticipate and respond to new risks as they arise.
  • Provide care delivery at any of the member sites as program needs dictate.
  • Provide strong and persuasive advocacy for member sites and their communities.
  • Supervise and maintain direct reports by providing direction, leadership, coaching, feedback, and training.
  • Implement the performance management process including regularly conducting direct reports’ performance reviews and proving development opportunities.
  • Participate in recruitment and selection and termination processes of staff except for director level.
  • Ensure delegation to direct reports is appropriate and supported.
  • Perform other duties as assigned to drive IPHCC’s Vision, Mission, and Touchstone Values.

Skills and Qualifications

  • Minimum bachelor’s degree in a Health-Related Discipline or an equivalent combination of training and/or relevant experience.
  • Master’s level degree or other postgraduate training in a related field is an asset.
  • Ten or more years of experience in Senior Management.
  • Experience managing various funding portfolios.
  • Experience working with diverse communities that reflect IPHCC’s priority populations.
  • Knowledge of the social determinants of health and health equity.
  • A solid background in community-based development, including experience in at least two of the following areas: primary healthcare, population health, youth and/or family services, support to racialized and/or vulnerable populations, community development, or community-based education programs.
  • Experience in generating funding for programs, including writing proposals and/or generating fee-for-service programs that enhance the sustainability of the programs.
  • Able to effectively communicate both verbally and in writing.
  • Able to obtain a successful vulnerable sector screen.

  • Capacity for sensitive and responsive listening and concentration skills, due to frequent interruptions, for short periods of time on a continuous basis.

  • Office environment - moderate-to-extensive work on computer keyboard and screen.
  • Multiple and simultaneous deadlines pressures requiring extended hours of focus.
