Reporting to the Vice President of Client Services, the Director, Community Health Services is responsible for the strategic oversight, guidance, and delivery of quality and efficient Community Health Services at Community Care Durham. The Director will collaborate with the Chief Executive Officer, Vice Presidents, Client Services, and Management teams to develop and implement strategies that enhance and improve care outcomes across all Community Health programs, including, In-Home Supports, Nursing, Adult Day Program, and FootCare.

Responsibilities include but not limited to:

  • Provides guidance and support in the safe, effective, and efficient delivery, and availability of client care and services, with a strong focus on continuous quality improvement;
  • Collaborates across departments to develop strategies and standardized policies and procedures on client care and service delivery;
  • Provides leadership and support in managing escalated client and caregiver concerns and client safety issues;
  • Promotes a culture of accountability by addressing concerns and complaints within the team, and ensuring feedback and learning opportunities are communicated;
  • Identifies opportunities to enhance existing policies, processes, and guidelines related to service and makes recommendations and implementation of new practices as required;
  • Assists in the implementation of best practices to support the delivery of safe, effective, and efficient quality services;
  • Ensures all employees in portfolio are monitored and evaluated for best practice and performance expectations;
  • Monitors MSAA target and statistical reports to manage financial performance;
  • Conducts performance reviews on direct reports and provides guidance on addressing Managers/Supervisors with coaching, mentoring and recognition strategies and opportunities;
  • Represents CCD at various external collaborations/committees (i.e., Lakeridge Health, Downtown Oshawa Project) as required within the scope of the role;
  • Fosters collaborative relationships with community partners (i.e., community support service agencies, hospitals, Ontario Health at Home, etc.);
  • Participates as part of on-call rotation to support continuity of service.

Qualifications required:

  • Post secondary education in Nursing
  • Registered Practical Nurse (RPN) in good standing (with no restrictions) with the College of Nurses of Ontario (CNO)
  • Membership with Registered Practical Nurses Association of Ontario (RPNAO) preferred
  • Minimum five (5) years' experience in a healthcare or community not-for-profit setting
  • Minimum five (5) years’ experience in a management role is required
  • Experience working with the vulnerable population with knowledge of community resources and funding sources
  • Proven knowledge and skills with client records management and electronic medical records and external reporting
  • Strong critical thinking skills and organizational/time management skills
  • Experience with budgets, meeting targets, and statistics
  • Self-directed and highly motivated with excellent interpersonal and communication skills (both written and verbal)
  • Excellent organizational and coordination skills with the ability to prioritize competing workload demands
  • Able to provide leadership, support, and critical analysis in stressful and emotional situations
  • Committed to innovation and continuous quality improvement
  • Proficient computer skills and experience using Microsoft Office (Word, Excel, SharePoint, etc.)
  • Valid Ontario driver’s “G” license with access to unlimited use of a vehicle
  • Sensitivity to the needs of the vulnerable population with disabilities, including challenges in cognition  
  • Clear Vulnerable Sector Police Check completed within the past 6 months. (Please note: 4–6 weeks processing time)
  • Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation and Benefits: $98,000 annually;

  • Three (3) weeks’ vacation per year of work (pro-rated to hours worked);
  • Six (6) paid sick days per year of work (pro-rated to hours worked);
  • Nine (9) paid (including stat) holidays;
  • Three (3) float days (when eligible);
  • Health, dental, life insurance, and pension benefits when eligible;
  • Performance incentive eligibility up to 5% of annual salary when eligible.
